摘要
目的探讨寒冷环境对创伤后机体免疫功能和炎症因子的影响。方法选取10只新西兰大白兔,随机分为常温组和低温组,每组5只。常温组兔置于22~25℃条件下,不另行处理;低温组兔置于-10~-15℃条件下冷刺激处理。观察常温组与低温组兔处理后0、2、4、8 h及复温后12、24 h临床症状及生理指标(体温、收缩压、舒张压、心率及血氧饱和度);检测兔各时间点炎症因子[白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肿瘤坏死因子α(TNF-α)]水平及免疫功能(CD3^(+)、B细胞、NK细胞、血小板计数、白细胞计数)情况。结果低温组建模2 h后兔轻微颤抖,创口渗血;建模4 h后兔全身颤抖,伤口周围出现散性小水泡;建模8 h后兔创面红肿、出现小冰晶,创伤皮肤变为紫褐色,肢体僵硬;复温后伤口出现血性分泌物,伤口周围皮肤呈暗红色。创伤后2、4、8 h,低温组兔的体温、收缩压、舒张压、心率低于常温组兔;复温12、24 h,低温组兔的体温低于常温组,差异有统计学意义(P<0.05)。创伤后2、4、8 h,低温组兔的IL-10、血小板计数水平低于常温组,IL-6和TNF-α水平高于常温组,差异有统计学意义(P<0.05)。创伤后2、4、8 h,低温组兔CD3+、B细胞、NK细胞水平低于常温组,白细胞计数水平高于常温组,差异有统计学意义(P<0.05)。结论寒冷环境可使免疫系统受到影响,炎症反应的调节能力减弱。
Objective To explore the influence of cold environment on the immune function and inflammatory factors after trauma.Methods A total of 10 New Zealand white rabbits were selected and randomly divided into normal temperature group and low temperature group,with 5 rabbits in each group.Normal temperature group rabbits were placed at 22 to 25℃without additional treatment;low temperature group rabbits were placed at-10 to-15℃for cold stimulation.Clinical symptoms and physiological indicators(body temperature,systolic,diastolic pressure,heart rate and oxygen saturation)were observed at 0,2,4 and 8 h after treatment,and 12 and 24 h after return temperature in normal temperature group and low temperature group.The level of inflammatory factors[interleukin-6(IL-6),interleukin-10(IL-10),tumor necrosis factorα(TNF-α)]and immune function(CD3^(+),B cells,NK cells,platelet count,white blood cell count)were detected at each time point.Results At 2,4 and 8 h after trauma,the body temperature,heart rate,systolic blood pressure and diastolic blood pressure in low temperature group were lower than the normal temperature group,the differences were statistically significant(P<0.05).At 12 and 24 h after return temperature,the body temperaturein low temperature group were lower than the normal temperature group,the differences were statistically significant(P<0.05).At 2,4 and 8 h after trauma,the levels of IL-10 and platelet count in low temperature group were lower than those in the normal temperature group,and the levels of interleukin-6 and tumor necrosis factor-αwere higher than those in the normal temperature group,the differences were statistically significant(P<0.05).At 2,4 and 8 h after trauma,the number of CD3+,B cells and NK cells in low temperature group was significantly less than normal temperature group,and the white blood cell level was higher than normal temperature group,the differences were statistically significant(P<0.05).Conclusion Cold environment can affect the immune system,and the regulation of the infla
