摘要
目的分析原发性肝癌患者经皮射频消融术后肝功能不全发生的危险因素。方法选取2018年1月至2023年1月在我院接受经皮射频消融术治疗的100例原发性肝癌患者,术后采用Child-Pugh分级方法进行肝功能分级,将Child-Pugh分级为C级的患者纳入肝功能不全组(n=20),其余患者纳入非肝功能不全组(n=80),采用单因素和多因素分析找出原发性肝癌患者经皮射频消融术后肝功能不全发生的相关危险因素。结果肝功能不全组与非肝功能不全组的临床分期、术前肝功能分级、消融范围比较差异有统计学意义(P<0.05);Logistic回归分析结果显示,术前肝功能分级、消融范围为原发性肝癌患者经皮射频消融术后肝功能不全发生的独立危险因素(P<0.05)。结论原发性肝癌患者经皮射频消融术后存在一定的肝功能不全发生风险,且术后肝功能不全的发生受患者术前肝功能分级及消融范围的影响。
Objective To analyze the risk factors for the occurrence of hepatic insufficiency after percutaneous radiofrequency ablation in patients with primary hepatocellular carcinoma.Methods 100 patients with primary hepatocellular carcinoma undergoing percutaneous radiofrequency ablation in our hospital from January 2018 to January 2023 were selected.The Child-Pugh grading method was used to classify liver function after operation.Patients with Child-Pugh grade C were included in the liver dysfunction group(n=20),and the remaining patients were included in the non-liver dysfunction group(n=80).Univariate and multivariate analysis were used to find out the related risk factors for the occurrence of hepatic insufficiency after percutaneous radiofrequency ablation in patients with primary hepatocellular carcinoma.Results Statistical difference was found in the clinical stage,preoperative liver function grading and ablation range between liver dysfunction group and non-liver dysfunction group(P<0.05);Logistic regression analysis showed that preoperative liver function grading and ablation range were independent risk factors for the occurrence of hepatic insufficiency after percutaneous radiofrequency ablation in patients with primary hepatocellular carcinoma(P<0.05).Conclusions There is a certain risk for the occurrence of hepatic insufficiency after percutaneous radiofrequency ablation in patients with primary hepatocellular carcinoma,and the occurrence of postoperative liver dysfunction is affected by the preoperative liver function classification and ablation range of patients.
