摘要
目的分析肾盂旁囊肿行超声引导下输尿管软镜钬激光内切开引流术治疗的临床疗效。方法选取我院2018年1月至2023年12月收治的56例肾盂旁囊肿患者展开研究,依据随机数字表法分为对照组与试验组(每组28例)。对照组给予腹腔镜下去顶减压术治疗,试验组超声引导下输尿管软镜钬激光内切开引流术治疗。观察两组治疗前后氧化应激反应[丙二醛(MDA)、过氧化氢酶(CAT)、谷胱甘肽(GSH)、超氧化物歧化酶(SOD)]、炎症因子水平[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、C反应蛋白(CRP)]、手术相关指标、疗效以及并发症。结果治疗后,试验组GSH、CAT、SOD水平高于对照组,MDA、TNF-α、IL-1β以及CRP水平低于对照组(P<0.05)。试验组手术时间、住院时间、肛门排气以及术后卧床时间均较对照组更短,术中出血量较对照组更少(P<0.05)。试验组与对照组治疗总有效率分别为96.43%、85.71%,并发症发生率分别为7.14%、17.86%,组间对比差异均无统计学意义(均P>0.05)。结论超声引导输尿管软镜钬激光内切开引流术和腹腔镜去顶减压术治疗肾盂旁囊肿均有效且安全,但超声引导下输尿管软镜钬激光内切开引流术治疗更利于降低患者应激反应与炎症反应,促进患者术后快速恢复。
Objective To analyze the efficacy of ultrasound-guided ureteroscopic holmium laser internal incision and drainage for the treatment of renal pelvis cysts.Methods A total of 56 patients with renal pelvis cysts admitted to our hospital from January 2018 to December 2023 were selected for the study.The patients were randomly divided into a control group and an experimental group using a random number table method,with 28 patients in each group.The control group was treated with laparoscopic decompression surgery,while the experimental group was treated with ultrasound-guided ureteroscopy holmium laser internal incision and drainage.Observe the oxidative stress response[malondialdehyde(MDA),catalase(CAT),glutathione(GSH),superoxide dismutase(SOD)],inflammatory factor levels[tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),C-reactive protein(CRP)],surgical related indicators,efficacy,and complications before and after treatment in two groups.Results After treatment,the levels of GSH,CAT,and SOD in the experimental group were higher than those in the control group,while the levels of MDA,TNF-α,IL-1β,and CRP were lower than those in the control group(P<0.05).Compared to the control group,the experimental group had shorter surgical time,anal exhaust time,postoperative bed rest time,and hospitalization time,and lower intraoperative bleeding volume(P<0.05).The total effective rate of treatment in the experimental group was 96.43%,slightly higher than the control group's 85.71%,while the incidence of complications was 7.14%,slightly lower than the control group's 17.86%,but the difference between the groups was not statistically significant(P>0.05).Conclusions In the treatment of renal pelvis cysts,ultrasound-guided ureteroscopy holmium laser internal incision and drainage and laparoscopic decompression have significant efficacy and safety.However,ultrasound-guided ureteroscopy holmium laser internal incision and drainage is more conducive to reducing patients'stress and inflammatory reactions and promoting rapid
