糖化血红蛋白检测对糖尿病并发症的预测价值

摘要 目的研究糖化血红蛋白(HbA1c)检测对糖尿病并发症的预测价值。方法选取2023年6月至2024年6月福清市阳下街道社区卫生服务中心收治的98例糖尿病患者作为研究对象。依据有无发生糖尿病并发症,将入选对象分为糖尿病并发症组、糖尿病无并发症组,对比两组基线资料及HbA1c、空腹血糖(FBG)、随机血糖(GLU)水平,并绘制受试者工作特征(ROC)曲线,计算曲线下面积(AUC)、灵敏度、特异度、约登指数、最佳阈值,以评估上述血糖指标水平对糖尿病并发症的预测价值。结果98例糖尿病患者发生及未发生糖尿病并发症的例数分别为23例、75例,占比分别为23.47%、76.53%,分别归为糖尿病并发症组、糖尿病无并发症组。糖尿病并发症组HbA1c、FBG、GLU水平均高于糖尿病无并发症组(P<0.05)。HbA1c AUC值(0.754)高于FBG、GLU(0.612、0.623),且AUC值均>0.6。结论糖尿病并发症患者HbA1c、FBG、GLU水平均高于糖尿病无并发症患者,且相较于FBG、GLU血糖指标水平,HbA1c对于糖尿病并发症的预测价值更优。 Objective To study the application effect of glycated hemoglobin(HbA1c)test in the prediction of diabetic complications.Methods Diabetic patients admitted to the Fuqing City Yangxia Street Community Health Service Center from June 2023 to June 2024 were selected as study subjects(n=98).Based on the presence or absence of diabetic complications,the enrolled subjects were divided into the diabetic complications group and the diabetes without complications group,comparing the baseline data and the levels of HbA1c,FBG,and GLU between the two groups,and plotting the subjects'work characteristics(ROC)curves,calculating the area under the curve(AUC),the sensitivity,the specificity,the Jordon's index,and the optimal threshold,in order to assess the level of the above glucose indexes on the diabetic complications predictive value.Results Among the enrolled subjects,23 and 75 patients,accounting for 23.47% and 76.53% of the total number of patients with and without diabetic complications were categorized into the diabetic complications group and the diabetes without complications group,respectively.The levels of HbA1c,FBG and GLU in the diabetes complications group were higher than those in the diabetes without complications group(P<0.05).The HbA1c AUC value(0.754)was higher than that of FBG and GLU(0.612,0.623),and the AUC values were all>0.6.Conclusions The levels of HbA1c,FBG,and GLU glycemic indexes were higher in patients with diabetic complications than in patients without diabetic complications,and the predictive value of HbA1c for diabetic complications was superior compared with the levels of FBG and GLU glycemic indexes.
