摘要
目的:观察缺血性脑卒中患者视网膜微血管神经改变特征。方法:选取2021年6月—2023年6月吉林省前卫医院收治的缺血性脑卒中患者40例作为研究对象,根据颈内动脉狭窄程度分为观察组(颈内动脉狭窄≥50%)与对照组(颈内动脉狭窄<50%),各20例。两组患者均接受眼科裂隙灯检查、眼底照相以及光学相干断层扫描血管成像检查。比较两组眼底病变情况、双侧黄斑区视网膜参数及视盘区视网膜参数。结果:观察组视网膜动静脉交叉征、视网膜动脉硬化、眼底棉絮斑、眼底出血斑、眼底微血管瘤、视网膜硬性渗出发生率均高于对照组(P<0.05)。观察组浅层毛细血管丛血流密度、深层毛细血管丛血流密度、黄斑中心凹无灌注区周围环血流密度、黄斑中心凹厚度和神经节细胞复合体厚度均小于对照组(P<0.05)。观察组视盘血流密度和视盘区视网膜神经纤维层厚度均小于对照组(P<0.05)。结论:缺血性脑卒中患者眼底病变发生风险较高,且随着颈内动脉狭窄程度加重,眼底病变发生风险升高,黄斑区和视盘区视网膜参数异常率增加。
Objective:To observe the changes of retinal microvascular nerve in patients with ischemic stroke.Methods:Forty pa⁃tients with ischemic stroke admitted to Jilin Qianwei Hospital from June 2021 to June 2023 were selected as the study objects.Ac⁃cording to the degree of internal carotid artery stenosis,they were divided into observation group(internal carotid artery stenosis≥50%)and control group(internal carotid artery stenosis<50%),with twenty cases each.Both groups underwent ophthalmic slitlamp examination,fundus photography,and optical coherence tomography angiography.Fundus lesions,retinal parameters in macular area and retinal parameters in optic disc area were compared between the two groups.Results:The incidence of retinal ar⁃teriovenous cross sign,retinal arteriosclerosis,fundus cotton spot,fundus blood spot,fundus microhemangioma and retinal rigid exudation in observation group were higher than those in control group(P<0.05);the blood flow density of superficial capillary plexus,deep capillary plexus,peripheral blood flow density of macular fovea without perfusion,thickness of macular fovea and ganglion cell complex in observation group were lower than those in control group(P<0.05);the blood flow density and retinal nerve fiber layer thickness in the observation group were lower than those in the control group(P<0.05).Conclusion:Patients with ischemic stroke had a higher risk of fundus lesions,and with the aggravation of internal carotid artery stenosis,the risk of fun⁃dus lesions increased,and the abnormal rate of retinal parameters in macular area and optic disc area increased.
