摘要
目的调查多发性骨髓瘤化疗患者心理韧性、应对方式和生活质量的现状,探讨三者的相关性。方法选取2022年5月18日至2023年6月20日衡阳市南华大学附属第一医院、南华大学附属第二医院、南华大学附属南华医院、湖南师范大学附属衡阳市中心医院、衡阳市第一人民医院住院治疗的220例多发性骨髓瘤患者作为研究对象,采用中文版Connor-Davidsion心理韧性量表、简易应对方式量表、欧洲癌症研究与治疗组织生活质量问卷进行问卷调查,统计患者的心理韧性、应对方式、生活质量各维度得分,并与Norwegian常模进行比较,采用Pearson相关分析探讨心理韧性、应对方式、生活质量三者的相关性。结果多发性骨髓瘤化疗患者心理韧性总均分为(47.56±7.19)分,低于量表总分的中间值。应对方式中积极维度条目均分为(1.55±0.42)分、消极维度条目均分为(1.44±0.37)分,积极维度得分高于消极维度得分。生活质量总得分为(56.45±9.34)分,低于Norwegian常模(75.3)。多发性骨髓瘤化疗患者心理韧性中坚韧、自强、乐观维度与积极应对呈正相关(r=0.427、r=0.445、r=0.331,P<0.05),坚韧维度与经济困难呈正相关(r=0.028,P<0.05),乐观维度与情绪功能呈正相关(r=0.032,P<0.05);积极应对维度与躯体功能、认知功能均呈负相关(r=-0.234,r=-0.308,P<0.05),与总健康状况呈正相关(r=0.231,P<0.05);消极应对维度与躯体功能、恶心呕吐呈正相关(r=0.243、r=0.265,P<0.05)。结论多发性骨髓瘤化疗患者心理韧性及生活质量均处于低水平,在患者护理中要采取积极的干预措施,提高患者的心理韧性水平,指导患者采取合适的应对方式,不断提高生活质量。
Objective To investigate the resilience,coping style and quality of life level in multiple myeloma chemotherapy patients,and to explore the correlation between the three.Methods A total of 220 multiple myeloma chemotherapy patients who were hospitalized in the First Affiliated Hospital of the University of South China,the Second Affiliated Hospital of the University of South China,the Nanhua Hospital of the University of South China,Hengyang Central Hospital of Hunan Normal University and the First People's Hospital of Hengyang City from May 18,2022 to June 20,2023 were selected as the study objects.The Chinese Connor-Davidson resilience scale,simplified coping style questionnaire,and European Organization for Research and Treatment of Cancer quality of life questionnaire were used to conduct a questionnaire survey,and the scores of resilience,coping style,and quality of life of patients were collected and compared with the Norwegian norm.Pearson correlation analysis was used to explore the correlation among resilience,coping style and quality of life.Results The total resilience score of patients with multiple myeloma chemotherapy was(47.56±7.19)points,which was lower than the median score of the scale.In coping styles,the positive dimension items were divided into(1.55±0.42)points,and the negative dimension items were divided into(1.44±0.37)points,and the positive dimension score was higher than the negative dimension score.The overall quality of life score(56.45±9.34)points was lower than the Norwegian norm(75.3)points.In the resilience of patients with multiple myeloma chemotherapy,tenacity,self-improvement and optimism were positively correlated with positive coping(r=0.427,r=0.445,r=0.331,P<0.05),tenacity was positively correlated with economic hardship(r=0.028,P<0.05),and optimism was positively correlated with emotional function(r=0.032,P<0.05).The positive coping dimension was negatively correlated with physical function and cognitive function(r=-0.234,r=-0.308,P<0.05),and positively correlated with
