摘要
结合相关文献和临床实践,对国内外失能老年人健康管理模式现状进行综述,主要介绍了基于社区的失能老年人健康管理模式、基于网络技术的失能老年人健康管理模式、医养结合健康管理模式及家庭医生制健康管理模式,并对现阶段失能老年人健康管理存在的不足与展望进行分析,旨在为失能老年人健康管理和合理干预提供借鉴与参考。
Based on relevant literatures and clinical practice,this paper reviewed the status quo of health management models for disabled elderly at home and abroad.It mainly introduced the community-based health management model for disabled elderly,the network technology based health management model for disabled elderly,the combination of medical and nursing health management model and the family doctor health management model.The paper also analyzed the shortcomings and prospects of health management of disabled elderly people,so as to provide reference for health management and reasonable intervention of disabled elderly people.
