摘要
目的探讨经尿道钬激光切除术治疗非肌层浸润性膀胱癌对患者手术相关指标及排尿功能的影响。方法选取2020年9月—2023年9月金乡县人民医院收治的92例非肌层浸润性膀胱癌患者为研究对象,按随机数字表法将其分为对照组和观察组,每组46例。对照组采用经尿道电切术治疗,观察组采用经尿道钬激光切除术治疗。对比两组患者的围手术期指标、尿流动力学指标、炎性因子指标、生活质量及并发症发生情况。结果观察组手术、尿管留置及住院时间均短于对照组,术中出血量少于对照组,一次性病灶完整切除率高于对照组,组间差异有统计学意义(P<0.05)。术后3个月,观察组残余尿量少于对照组,膀胱顺应性水平高于对照组,最大尿流率快于对照组,组间差异有统计学意义(P<0.05);术后3 d,观察组白细胞介素-6、C反应蛋白、肿瘤坏死因子-α水平均低于对照组,组间差异有统计学意义(P<0.05);术后1个月,观察组生活质量综合评定问卷中躯体功能、心理功能、社会功能、物质生活状态评分均高于对照组,组间差异有统计学意义(P<0.05)。观察组并发症发生率为4.17%,低于对照组的18.75%,差异有统计学意义(P<0.05)。结论经尿道钬激光切除术治疗非肌层浸润性膀胱癌存在更小的损伤、更少的并发症,能够减轻患者炎症反应,加快其排尿功能的恢复,提高患者生活质量。
Objective Objective To investigate the effects of transurethral holmium laser resection for non-muscle-invasive bladder cancer on operative indexes and urination function.Methods Ninety-two patients with non-muscle-invasive bladder cancer admitted to Jinxiang County People's Hospital from September 2020 to September 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 46 cases in each group.The control group was treated with transurethral resection,and the observation group was treated with transurethral holmium laser resection.Perioperative indicators,urodynamics indicators,inflammatory factors indicators,quality of life and complications were compared between the two groups.Results The operative time,urinary catheter retention time and hospital stay in the observation group were shorter than those in the control group,the intraoperative blood loss was less than that in the control group,and the complete resection rate of one-time lesions was higher than that in the control group,and the differences between the groups were statistically significant(P<0.05);3 months after operation,the residual urine volume of observation group was lower than that of control group,the bladder compliance level was higher than that of control group,and the maximum urine flow rate was faster than that of control group,and the differences between the groups were statistically significant(P<0.05);3 days after operation,the levels of interleukin-6,C-reactive protein and tumor necrosis factor-αin the observation group were lower than those in the control group,and the differences between the groups were statistically significant(P<0.05);1 month after operation,the scores of physical function,psychological function,social function and material life status in Generic Quality of Life Inventory-74 were higher than those in the control group,and the differences between the groups were statistically significant(P<0.05).The complication rate in t
