摘要
目的探讨予以老年2型糖尿病(T2DM)患者二甲双胍联合德谷胰岛素治疗的效果。方法选择2021年4月—2023年5月吉林市人民医院收治的86例老年T2DM患者为研究对象,按随机数字表法将其分为对照组和观察组,每组43例。对照组采用二甲双胍治疗,观察组在对照组基础上联合德谷胰岛素治疗,两组均持续治疗12周。对比两组患者的血糖水平、体质指数、胰岛功能及不良反应发生情况。结果治疗后,观察组腹血糖、餐后2 h血糖及糖化血红蛋白水平分别为(7.05±0.98)mmol/L、(8.42±1.35)mmol/L、(7.35±1.28)%,均低于对照组的(8.24±1.13)mmol/L、(9.97±1.43)mmol/L、(8.06±1.43)%,组间差异有统计学意义(P<0.05);观察组体质指数为(25.28±1.77)kg/m^(2),低于对照组的(26.25±2.05)kg/m^(2),差异有统计学意义(P<0.05);观察组空腹胰岛素水平为(8.84±1.29)mU/L,高于对照组的(7.92±1.24)mU/L,胰岛素抵抗指数为(2.95±0.37),低于对照组的(3.52±0.40),组间差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论德谷胰岛素联合二甲双胍治疗在老年T2DM患者中具有较好的应用效果,可降低血糖水平与体质指数,改善胰岛功能,且具有较好的应用安全性。
Objective To investigate the effect of metformin combined with Degu insulin in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of 86 elderly T2DM patients admitted to Jilin City People's Hospital from April 2021 to May 2023 were selected as the research objects,and were divided into a control group and an observation group according to random number table method,with 43 cases in each group.The control group was treated with metformin,and the observation group was treated with Degu insulin on the basis of the control group,both groups were treated for 12 weeks.Blood glucose level,body mass index,islet function and the occurrence of adverse reactions were compared between the two groups.Results After treatment,the levels of abdominal blood glucose,2 hours postprandial blood glucose and HBA1c in the observation group were(7.05±0.98)mmol/L,(8.42±1.35)mmol/L and(7.35±1.28)%,respectively,which were lower than(8.24±1.13)mmol/L,(9.97±1.43)mmol/L and(8.06±1.43)%in the control group,and the differences between the groups were statistically significant(P<0.05);the body mass index of observation group was(25.28±1.77)kg/m^(2),which was lower than that of the control group(26.25±2.05)kg/m^(2),and the difference was statistically significant(P<0.05);the fasting insulin level in the observation group was(8.84±1.29)mU/L,which was higher than(7.92±1.24)mU/L in the control group,and the insulin resistance index was(2.95±0.37),which was lower than(3.52±0.40)in the control group,and the differences between the groups were statistical significance(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The treatment of Degu insulin combined with metformin has a good application effect in elderly T2DM patients,which can reduce blood glucose level and body mass index,improve islet function,and has a good application safety.
