摘要
目的探究术前超声在甲状腺癌颈侧淋巴结转移预测中PI、TTP及MTT的临床意义。方法抽取我院2022年2月-2023年10月期间收治的92例甲状腺癌患者开展研究,根据是否发生颈部淋巴结转移分组:发生颈部淋巴结转移患者为A组(n=35)、未发生颈部淋巴结转移患者为B组(n=57),于术前进行超声检查(常规超声、超声造影),对比两组常规超声影像学表现及造影超声参数[达峰时间(time to peak,TTP)、峰值强度(peak intensity,PI)、造影剂平均通过时间(mean transit time,MTT)]。另外,绘制ROC曲线分析微钙化、TTP、PI及MTT预测甲状腺癌颈侧淋巴结转移的诊断效能。结果A组有微钙化占比(82.86%)较B组(61.40%)高(P<0.05);A组PI、TTP及MTT低于B组(P<0.05);绘制ROC,PI、TTP及MTT预测甲状腺癌颈侧淋巴结转移的AUC面积高于微钙化。结论微钙化、TTP、PI及MTT均能预测甲状腺癌颈侧淋巴结转移,但PI、TTP及MTT预测价值更高,能为其手术治疗提供客观的参考依据。
Objective To explore the clinical significance of preoperative ultrasound in predicting cervical lymph node metastasis in thyroid cancer,including PI,TTP,and MTT.Methods A total of 92 thyroid cancer patients admitted to our hospital from February 2022 to October 2023 were selected for study.They were divided into two groups based on whether cervical lymph node metastasis occurred:Group A(n=35)had cervical lymph node metastasis,and Group B(n=57)had no cervical lymph node metastasis.Ultrasound examination(conventional ultrasound,contrast-enhanced ultrasound)was performed before surgery,and the conventional ultrasound imaging manifestations and contrast-enhanced ultrasound parameters[time to peak(TTP),peak intensity(PI),and mean transit time(MTT)of the contrast agent]were compared between the two groups.In addition,drawing ROC curves to analyze the diagnostic efficacy of microcalcification,TTP,PI,and MTT in predicting cervical lymph node metastasis in thyroid cancer.Results The proportion of microcalcifications in Group A(82.86%)was higher than that in Group B(61.40%)(P<0.05);the PI,TTP,and MTT of Group A were lower than those of Group B(P<0.05);draw ROC,PI,TTP,and MTT to predict the AUC area of lateral neck lymph node metastasis in thyroid cancer,which is higher than microcalcification.Conclusion Microcalcification,TTP,PI,and MTT can all predict cervical lymph node metastasis in thyroid cancer,but PI,TTP,and MTT have higher predictive value and can provide objective reference for surgical treatment.
