967名海军飞行人员甲状腺疾病调查分析

Investigation and analysis of thyroid diseases in 967 naval pilots

摘要 目的研究海军飞行人员年度体检甲状腺疾病患病情况,探讨飞行人员甲状腺疾病分布情况及发病特征。方法选取2019年1月至2021年12月在空军杭州特勤疗养中心疗养三区行年度体检鉴定的967名海军飞行人员,回顾性统计分析甲状腺疾病患病情况。结果在967名海军飞行人员中,甲状腺结节是其罹患的主要病种,共计306例;其中,3例为甲状腺乳头状癌。随着年龄、飞行时间的增长,甲状腺结节检出率增加(P<0.01);同时,随着年龄的增长,C-TIRADS 2类甲状腺结节占比下降,C-TIRADS 3类、4类甲状腺结节占比上升(P<0.001)。此外,检出弥漫性病变21例,其中6例为桥本甲状腺炎。检出甲状腺功能异常53例,其中临床甲亢1例,亚临床甲亢5例,亚临床甲减10例,各年龄组甲状腺功能指标异常检出率差异无统计学意义(P>0.05)。结论海军飞行人员是甲状腺疾病高发人群,主要涉及结节性病变、炎症性病变及甲状腺功能异常,尤其是甲状腺结节检出率较高。应完善海军飞行人员年度体检甲状腺疾病筛查项目,科学制定甲状腺疾病管理路径,建立并落实动态随访机制,提高航空卫生保障能力,维护飞行人员身体健康。 Objective To investigate thyroid diseases’prevalence in annual medical examinations of naval pilots,and study the distribution and characteristics of thyroid diseases in those pilots.Methods A total of 967 naval pilots who underwent annual physical and medical examinations and evaluations in Hangzhou Rehabilitation Center for Special Duty Servicemen of the PLA Air Force from January 2019 to December 2021 were selected as research subjects,and their thyroid diseases’prevalence was retrospectively analyzed.Results Among the 967 naval pilots,thyroid nodules were the main disease,with a total of 306 cases;among them,three cases were papillary thyroid carcinoma.The detection rate of thyroid nodules increases with age and flight time(P<0.01).Meanwhile,as age increases,the proportion of C-TIRADS type 2 thyroid nodules decreased,while the proportion of C-TIRADS type 3 and 4 thyroid nodules increased(P<0.001).In addition,21 cases of diffuse lesions were detected,of which 6 cases were Hashimoto’s thyroiditis.Fifty-three cases of thyroid dysfunction were detected,including 1 case of clinical hyperthyroidism,5 cases of subclinical hyperthyroidism,and 10 cases of subclinical hypothyroidism.There was no significant difference in the detection rate of abnormal thyroid function indicators among different age groups(P>0.05).Conclusion In this study,naval pilots were a high-risk group for thyroid diseases,mainly involving nodular lesions,inflammatory lesions,and thyroid dysfunction,especially with a high detection rate of thyroid nodules.It is suggested to improve the annual thyroid disease screening for naval pilots,scientifically develop a thyroid disease pathway management,establish and implement a dynamic follow-up mechanism,improve aviation health support capabilities,thus maintaining the physical health of pilots.
