摘要
目的探讨肺超声评分(LUS)联合血清长链非编码RNA(lncRNA)肺腺癌转移相关转录因子1(MALAT1),ln‐cRNA核富含丰富的转录本1(NEAT1)水平对新生儿呼吸窘迫综合征(NRDS)病情程度的诊断价值。方法选取在我院出生的82例NRDS新生儿作为NRDS组,并将其分为轻度组(41例)、中度组(31例)、重度组(10例)。另以我院同期健康的85例新生儿作为对照组,两组新生儿均接受肺部超声检查。采用实时荧光定量PCR检测血清MALAT1、NEAT1表达水平。采用受试者工作特征曲线(ROC)评估LUS联合MALAT1、NEAT1对新生儿中度、重度NRDS的预测价值。Spearman分析NEAT1、MALAT1与LUS相关性。结果与对照组相比,NRDS组左肺、右肺、双侧肺、双肺LUS及血清NEAT1、MALAT1相对表达水平较高,差异有统计学意义(P<0.05);中度组LUS、血清NEAT1、MALAT1相对表达水平高于轻度组,差异有统计学意义(P<0.05);重度组LUS、血清NEAT1、MALAT1相对表达水平高于中度组、轻度组,差异有统计学意义(P<0.05)。LUS联合MALAT1、NEAT1预测中度、重度NRDS的曲线下面积(AUC)分别为0.969、0.965。LUS与血清NEAT1、MALAT1水平呈正相关(P<0.05)。结论LUS及NEAT1、MALAT1表达水平随NRDS患儿病情程度增加而增加,LUS联合MALAT1、NEAT1对中度、重度NRDS有预测价值。
Objective To investigate and analyze the diagnostic value of lung ultrasound score(LUS)combined with serum long non-coding RNA(lncRNA)metastasis-associated lung adenocarcinoma transcript 1(MALAT1)and nuclear-enriched abundant transcript 1(NEAT1)levels for the severity of neonatal respiratory distress syndrome(NRDS).Methods Eighty-two NRDS neonates delivered in our hospital were selected as the NRDS group and were categorized into mild group(41 cases),moderate group(31 cases),and severe group(10 cases)with reference to the Berlin definition.85 healthy premature neonates in our hospital were regarded as the control group.All neonates in the group underwent lung ultrasonography.The expression levels of MALAT1 and NEAT1 in serum were detected by real-time fluorescence quantitative PCR.Receiver operating characteristic curve(ROC)was used to evaluate the predictive value of LUS combined with MALAT1 and NEAT1 in neonatal moderate and severe NRDS.Spearman was used to analyze the correlation between NEAT1,MALAT1 and LUS.Results Compared with the control group,the left lung,right lung,bilateral lung bases,bilateral lungs,both lungs and the relative expression levels of NEAT1 and MALAT1 were higher in the NRDS group(P<0.05).The LUS and the relative expression levels of serum NEAT1 and MALAT1 in the moderate group were higher than those in the mild group(P<0.05),the LUS and the relative expression levels of serum NEAT1 and MALAT1 in the severe group were higher than those in the moderate and mild groups(P<0.05).The area under the curve(AUC)of the LUS combined with MALAT1 and NEAT1 to predict moderate and severe NRDS was 0.969 and 0.965,respectively.LUS was positively correlated with serum NEAT1 and MALAT1 levels(P<0.05).Conclusion LUS and expression levels of NEAT1 and MALAT1 increase with the severity of NRDS in children with NRDS.LUS combined with MALAT1 and NEAT1 has predictive value for moderate and severe NRDS.
