摘要
目的:分析妇科临床麻醉经喉罩全麻通气的应用价值及安全性。方法:选取2022年8月—2023年8月贵航贵阳医院收治的妇科手术患者160例为研究对象,随机均分为对照组和观察组,每组80例。对照组进行插管麻醉,观察组进行经喉罩全麻通气,对比血流动力学指标、麻醉效果、围术期氧化应激指标。结果:建立气腹后30min、拔管后3min、手术结束时,观察组平均动脉压、心率水平低于对照组(P<0.05)。观察组麻醉起效时间、自主呼吸恢复时间、苏醒时间、拔管时间短于对照组(P<0.001)。术后1 d,观察组脂质过氧化产物(LHP)、谷胱甘肽过氧化物酶(GSH-PX)水平低于对照组(P<0.001);术后3d,观察组LHP水平低于对照组(P<0.001),两组GSH-PX水平比较,差异无统计学意义(P>0.05)。结论:妇科临床麻醉经喉罩全麻通气可以改善妇科手术患者血流动力学指标、围术期氧化应激指标,提高麻醉效果。
Objective:To analyze the application value and safety of gynecological clinical anesthesia through laryngeal mask general anesthesia ventilation.Methods:A total of one hundred and sixty gynecological surgery patients admitted to Guihang Guiyang Hospital from August 2022 to August 2023 were selected as the study objects,and were randomly divided into control group and observation group,with eighty cases in each group.The control group underwent intubation anesthesia,and the observation group underwent general anesthesia through laryngeal mask.Hemodynamic indexes,anesthesia effect and perioperative oxidative stress indexes were compared.Results:At 30 min after pneumoperitoneum establishment,3 min after extubation and the end of operation,the mean arterial pressure and heart rate of observation group were lower than those of control group(P<0.05);the effective time of anesthesia,recovery time of spontaneous breathing,recovery time of awakening and extubation time of observation group were shorter than those of control group(P<0.001);one day after operation,the levels of lipid peroxidation(LHP)and glutathione peroxidase(GSH-PX)in observation group were lower than those in control group(P<0.001);three days after surgery,the LHP level in the observation group was lower than that in the control group(P<0.001),and there was no significant difference in GSH-PX level between the two groups(P>0.05).Conclusion:Gynecological clinical anesthesia through laryngeal mask general anesthesia can improve the hemodynamic index and perioperative oxidative stress index of gynecological surgery patients,and improve the anesthetic effect.
