Th22细胞及IL-22在儿童过敏性紫癜肾炎中的研究进展

摘要 过敏性紫癜(HSP)是儿童期最常见的系统性小血管炎,临床上通常表现为皮肤紫癜、消化道损伤、关节肿痛及肾脏损伤,近年来发病率呈增长趋势,部分患儿可出现肾脏受累,被称为过敏性紫癜肾炎(HSPN),肾脏的受累率为30%~50%,多发生在皮疹出现后的2~8周,有1%~7%的患儿会发展为终末期肾病,肾脏受累影响着远期预后,因此肾脏损害是影响HSP预后的关键因素。目前,HSP的病因和发病机制尚未完全明确,可能与机体感染、过敏反应相关,也可能涉及Th22细胞和白介素-22(Interleukin-22,IL-22)细胞因子参与的免疫反应、炎症反应以及由此导致的血管内皮损伤等因素。儿童过敏性紫癜肾炎对患儿的生存质量和预后构成严重威胁,因此,通过早期发现肾损害并降低其发病率具有重要意义。现就Th22细胞及IL-22在儿童过敏性紫癜肾炎中的研究进展作一简单综述,为后续HSP的诊断及治疗提供临床指导。 Henoch Schonlein purpura(HSP)is the most common systemic vasculitis fo children,with clinical manifestations of skin purpura,digestive tract injury,joint swelling and pains,and kidney damages.In recent years,its incidence has been increasing.Some children may have kidney involvement,which is called Henoch Schonlein purpura nephritis(HSPN).Incidence of kidney involvement is 30%to 50%,mostly occurs 2 to 8 weeks after appearance of rash.1%to 7%of children may develop end-stage renal disease,and kidney involvement affects long-term prognosis.Therefore,kidney damage is a key factor affecting prognosis of HSP.Etiology and pathogenesis of HSP is not completely clear yet,which may be related to infection and allergic reaction of the body,immune reaction,inflammatory reaction and vascular endothelial damage caused by Th22 cells and interleukin-22(IL-22)cytokines.HSPN seriously threatens life quality and prognosis of children.Research finds that early detection of renal damage can reduce incidence rate.The paper reviews research progress of Th22 cells and IL-22 for children with Henoch-Schonlein purpura nephritis briefly,provides clinical guidance for diagnosis and treatment of HSP in future.
