^(18)F-脱氧葡萄糖PET/CT与增强CT在胰腺癌鉴别诊断中应用价值观察

摘要 目的 分析胰腺癌诊断中应用^(18)F-脱氧葡萄糖(^(18)F-FDG)正电子发射断层成像(PET)/计算机断层扫描(CT)以及增强CT的应用价值。方法 选取2021年3月—2023年3月甘肃武威肿瘤医院收治的98例疑似胰腺癌患者为研究对象,并根据肿瘤直径将患者分为A组(>2cm)、B组(≤2cm),比较两组的影像特征。结果 经手术病理确诊阳性72例,阴性26例;^(18)F-FDG PET/CT诊断中阳性69例,阴性29例;增强CT诊断中阳性60例,阴性38例;联合诊断中阳性73例,阴性25例;联合诊断的灵敏度、特异度、准确度均明显高于^(18)F-FDG PET/CT、增强CT(P<0.05);确诊72例胰腺癌患者,经评估A组42例,B组30例,A组患者的CT值更高,早期及延迟SUVmax更高,远处转移率、边界不清率、胰腺形态异常率更高,与B组患者对比,差异有统计学意义(P<0.05)。结论 ^(18)F-FDG PET/CT联合增强CT的诊断效能更高,且小胰腺癌的影像表现有一定特殊性,应注意诊断鉴别。 Objective To analyze value of ^(18)F-FDG positron emission tomography(PET)/computed tomography(CT)and enhanced CT in diagnosis of pancreatic cancer.Methods The paper chose 98 patients with suspected pancreatic cancer in Wuwei Tumour Hospital from March 2021 to March 2023,and divided them into group A(>2cm)and group B(≤2cm)according to maximum diameter of tumor.Imaging manifestations between two groups were compared.Results 72 cases were confirmed positive and 26 cases negative by surgical pathology;69 cases were positive and 29 cases negative by ^(18)F-FDG PET/CT diagnosis.60 cases were positive and 38 cases negative by Enhanced CT diagnosis.73 cases were positive and 25 cases diagnosed negative by combined diagnosis.Sensitivity,specificity,and accuracy of combined diagnosis were significantly higher than ^(18)F-FDG PET/CT and contrast-enhanced CT,with significant difference(P<0.05).72 cases with pancreatic cancer were diagnosed.After evaluation,there were 42 cases in group A and 30 cases in group B.CT values,early and delayed SUVmax,distant metastasis rate,unclear border rate and abnormal pancreatic morphology rate in group A were higher than group B,with significant difference,(P<0.05).Conclusion ^(18)F-FDG PET/CT combined with enhanced CT can achieve high diagnostic efficiency for small pancreatic cancer,with some certain particularity for imaging manifestations of small pancreatic cancer,which should be paid and attention during diagnosis and differentiation.
