摘要
目的:探讨增强CT定量参数在大肠癌脉管癌栓评估中的应用效果。方法:回顾性分析2021年1月—2023年1月湖北省中西医结合医院收治的80例大肠癌患者临床资料,所有患者均行增强CT检查。观察腹部增强CT的影像学表现;比较脉管癌栓阳性患者与阴性患者的增强CT参数(ΔPMean、ΔPMax、ΔPMin、Tdia、Tvol值),评估增强CT定量参数与脉管癌栓的关系。结果:脉管癌栓阳性患者的ΔPMean、Tdia、Tvol值均明显高于脉管癌栓阴性患者,差异有统计学意义(t=4.166、5.247、5.817,P<0.05);Spearman分析显示,增强CT定量参数ΔPMean、Tdia、Tvol与大肠癌脉管癌栓阳性呈正相关(r=0.480、0.390、0.434,P<0.05);ROC曲线结果显示,ΔPMean、Tdia、Tvol值在评估大肠癌合并脉管癌栓中具有一定的诊断价值,且三指标联合评估大肠癌合并脉管癌栓的AUC明显高于各单项指标。结论:增强CT定量参数检查可用于筛查大肠癌是否合并脉管癌栓,在临床评估中具有一定参考价值。
Objective:To evaluate the value of quantitative parameters of contrast-enhanced CT in the evaluation of vascular tumor thrombus in colorectal cancer.Methods:The clinical data of 80 patients diagnosised with colorectal cancer from January 2021 to January 2022 were analyzed retrospectively.All patients underwent enhanced CT examination.The imaging findings of abdominal enhanced CT were observed,the enhanced CT parameters(ΔPMean,ΔPMax,ΔPMin,Tdia and Tvol)of patients with positive and negative vascular tumor thrombus were compared,and the relationship between the enhanced CT quantitative parameters and vascular tumor thrombus were evaluated.Results:TheΔPMean,Tdia and Tvol values of patients with positive vascular tumor thrombus were significantly higher than those of patients with negative vascular tumor thrombus(t=4.166,5.247,5.817;P<0.05).The quantitative parametersΔPMean,Tdia and Tvol of enhanced CT were positively correlated with the positive expression of vascular tumor thrombus in colorectal cancer(r=0.480,0.390,0.434;P<0.05).ROC showed thatΔPMean,Tdia and Tvol had certain diagnostic value in evaluating the vascular tumor thrombus in colorectal cancer,and AUC of the combined evaluation of the three indexes was significantly higher than that of each single index.Conclusion:Quantitative parameter of enhanced CT can be used to screen the presence of vascular tumor thrombus in colorectal cancer with certain reference value in clinical evaluation.
