摘要
目的探究妊娠期糖尿病(gestational diabetes mellitus,GDM)孕妇治疗中门冬胰岛素基础上加入地特胰岛素的应用效果。方法选取2022年1月—2024年6月江苏省连云港市赣榆区人民医院收治的80例GDM患者为研究对象,根据不同治疗方式分为两组,各40例。对照组采用门冬胰岛素治疗,研究组采用门冬胰岛素与地特胰岛素联合治疗。比较两组血糖水平、胰岛功能及不良妊娠结局发生率。结果治疗前,两组血糖水平、胰岛功能比较,差异无统计学意义(P均>0.05)。治疗后,研究组餐后2 h血糖、空腹血糖、糖化血红蛋白、胰岛素抵抗指数均低于对照组,胰岛β细胞功能指数高于对照组,差异有统计学意义(P均<0.05)。两组不良妊娠结局发生率比较,差异无统计学意义(P>0.05)。结论GDM孕妇治疗中采用门冬胰岛素与地特胰岛素联合治疗方法,控糖效果显著,可改善胰岛功能且不会增加不良妊娠结局发生率。
Objective To explore the application effect of insulin detemir on the basis of insulin aspart in the treatment of pregnant women with gestational diabetes mellitus(GDM).Methods A total of 80 GDM patients admitted to Ganyu District People's Hospital from January 2022 to June 2024 were selected as the research objects.According to different treatment methods,they were divided into two groups,with 40 cases in each group.The control group was treated with insulin aspart,and the study group was treated with insulin aspart combined with insulin detemir.The blood glucose levels,islet function and incidence of adverse pregnancy outcomes were compared between the two groups.Results Before treatment,there were no significant differences in blood glucose level and islet function between the two groups(all P>0.05).After treatment,the 2-hour postprandial blood glucose,fasting blood glucose,glycated hemoglobin A1c and insulin resistance index of the study group were lower than those of the control group,and the isletβcell function index was higher than that of the control group,the differences were statistically significant(all P<0.05).There was no significant difference in the incidence of adverse pregnancy outcomes between the two groups(P>0.05).Conclusion The combination of insulin aspart and insulin detemir in the treatment of GDM pregnant women has a significant effect on glucose control,which can improve islet function and will not increase the probability of adverse pregnancy outcomes.
