摘要
目的分析糖尿病(diabetes mellitus,DM)合并肾功能衰竭(renal failure,RF)患者联合使用胱抑素C(cystatin C,CysC)、24 h尿蛋白(24-hour urine protein,24 hUP)诊断的价值。方法选择2022年5月—2023年5月泰安市肿瘤防治院和淄博市中心医院收治的35例DM合并RF患者(B组)以及同期的35例单纯DM患者(A组)、35名健康体检者(C组)为研究对象,对比3组CysC、24 hUP水平以及分析二者联合检测的诊断效能。结果B组CysC、24 hUP水平均高于A组、C组,且A组CysC、24 hUP水平均高于C组,差异有统计学意义(P均<0.05)。CysC、24 hUP联合诊断确诊DM合并RF患者33例,联合诊断的灵敏度为94.29%(33/35)、特异度为87.14%(61/70)、准确度为89.52%(94/105)、阳性预测值为78.57%(33/42)、阴性预测值为96.83%(61/63),kappa值为0.776。结论CysC、24 hUP联合诊断DM合并RF,能提高诊断的准确度和可靠性,可为后续治疗提供科学的参考依据。
Objective To analyze the diagnostic value of combination of cystatin C(CysC)and 24-hour urine protein(24 hUP)in patients with diabetes mellitus(DM)complicated with renal failure(RF).Methods From May 2022 to May 2023,35 patients with DM complicated with RF(group B),35 patients with simple DM(group A)and 35 healthy subjects(group C)admitted to Tai'an Cancer Prevention and Control Institute and Zibo Central Hospital were selected as the research objects.The levels of CysC and 24 hUP in the three groups were compared and the diagnostic efficacy of the combined detection of the two indexs was analyzed.Results The levels of CysC and 24 hUP in group B were higher than those in groups A and C,and the levels of CysC and 24 hUP in group A were higher than those in group C,the differences were statistically significant(all P<0.05).33 patients with DM complicated with RF were diagnosed with CysC and 24 hUP.The sensitivity of combined diagnosis was 94.29%(33/35),specificity was 87.14%(61/70),ac⁃curacy was 89.52%(94/105),positive predictive value was 78.57%(33/42),negative predictive value was 96.83%(61/63),and kappa value was 0.776.Conclusion The combination of CysC and 24 hUP in the diagnosis of DM compli⁃cated with RF can improve the accuracy and reliability of diagnosis,and can provide scientific reference for follow-up treatment.
