摘要
目的探讨慢性阻塞性肺疾病(简称慢阻肺)伴急性呼吸衰竭患者联合应用电子支气管镜肺泡灌洗治疗、机械通气序贯治疗的临床效果。方法选取2020年1月—2024年1月阿克苏地区第一人民医院收治的120例慢阻肺伴急性呼吸衰竭患者为研究对象,按治疗方法不同分为两组,各60例。对照组给予机械通气序贯治疗,观察组给予机械通气序贯治疗同时应用电子支气管镜肺泡灌洗治疗。比较两组临床疗效、总通气时间、总住院治疗时间、二氧化碳分压(partial pressure of carbon dioxide,PaCO_(2))、血氧分压(partial pressure of blood oxygen,PaO_(2))、呼气峰值流量(peak expiratory flow,PEF)、肺总量(total lung capacity,TLC)、C反应蛋白(C-reactive protein,CRP)、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-8(interleukin-8,IL-8)、降钙素原(procalcitonin,PCT)水平。结果观察组治疗总有效率为93.33%(56/60),高于对照组的78.33%(47/60),差异有统计学意义(χ^(2)=5.551,P=0.018)。观察组总通气时间、总住院治疗时间短于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组PaCO_(2)低于对照组,PaO_(2)、PEF、TLC高于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组患者CRP、IL-6、IL-8、PCT水平低于对照组,差异有统计学意义(P均<0.05)。结论慢阻肺伴急性呼吸衰竭患者经机械通气序贯治疗基础上,与电子支气管镜肺泡灌洗联合应用效果确切,能够进一步缩短机械通气时间,改善血气及肺功能指标水平,促进炎症因子表达下调。
Objective To explor the clinical effect of combined application of electronic bronchoscopy alveolar lavage and mechanical ventilation sequential therapy in patients with chronic obstructive pulmonary disease(COPD)complicated with acute respiratory failure.Methods A total of 120 patients with chronic obstructive pulmonary disease and acute respiratory failure admitted to the First People's Hospital of Aksu Prefecture from January 2020 to January 2024 were selected as the research objects and divided into two groups according to different treatment methods,with 60 cases in each group.The control group was given sequential treatment of mechanical ventilation,and the observation group was given sequential treatment of mechanical ventilation combined with electronic bronchoscopy alveolar lavage.The clinical efficacy,total ventilation time,total hospitalization time,partial pressure of carbon dioxide(PaCO_(2)),partial pressure of blood oxygen(PaO_(2)),peak expiratory flow(PEF),total lung capacity(TLC),C-reactive protein(CRP),interleukin-6(IL-6),interleukin-8(IL-8),procalcitonin(PCT)were compared between the two groups.Results The total effective rate of treatment in the observation group was 93.33%(56/60),which was higher than 78.33%(47/60)of the control group,and the difference was statistically significant(χ^(2)=5.551,P=0.018).The total ventilation time and total hospitalization time in the observation group were shorter than those in the control group,and the differences were statistically significant(both P<0.05).After treatment,the PaCO_(2) of the observation group was lower than control group,the levles of PaO_(2),PEF and TLC were higher than those in the control group,the differences were statistically significant(all P<0.05).After treatment,the levels of CRP,IL-6,IL-8 and PCT in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).Conclusion On the basis of sequential treatment of mechanical ventilation in patients with chronic obstruct
