摘要
目的:分析胸腔镜和机器人肺癌手术住院费用变化及主要影响因素,为合理开展医疗费用控制提供参考。方法:采用描述性分析方法,对2021-2023年胸腔镜和机器人肺癌手术住院费用及住院费用构成情况进行统计,采用灰色关联分析方法对住院费用关联度进行分析。结果:2021-2023年,胸腔镜肺癌手术住院费用先减少后增加,机器人肺癌手术的住院费用逐年降低。两类手术患者住院费用构成前五位均为耗材费、诊断费、治疗费、综合医疗服务费和西药费,两类手术患者的综合医疗服务费、诊断费、治疗费均逐年升高,西药费和耗材费均呈下降趋势;两类手术患者住院费用中关联最大的均为耗材费。结论:胸腔镜和机器人肺癌手术住院费用相对合理,但费用结构仍需优化,耗材费对住院费用的影响最大,建议进一步加强医用耗材管理,同时建议完善肺癌手术临床路径,提高体现医务人员技术价值的医疗服务项目费用比例。
Objective:To analyze the changes of hospitalization expenses and the main influencing factors of video-assisted thoraco⁃scopic surgery(VATS)and robot-assisted thoracic surgery(RATS)for lung cancer,and provide reference for reasonable control of medical expenses.Methods:Descriptive analysis was used to analyze hospitalization expenses and composition of hospitalization expenses of VATS and RATS for lung cancer from 2021 to 2023.Grey correlation analysis was used to analyze the correlation between hospitaliza⁃tion expenses for the two types of surgery.Results:From 2021 to 2023,the hospitalization expenses of VATS for lung cancer decreased firstly and then increased,while the hospitalization expenses of RATS for lung cancer decreased year by year.The top five components of hospitalization expenses for the two types of surgery were medical consumables fees,diagnostic fees,treatment fees,comprehensive medical service fees,and western medicine fees.From a trend perspective,the comprehensive medical service fees,diagnostic fees,and treatment fees for the two types of surgery increased year by year,while western medicine fees and medical consumables fees had shown a downward trend.The most closely related hospitalization expenses for the two types of surgery was medical consumables fees.Conclusion:The hospitalization expenses of VATS and RATS for lung cancer is relatively reasonable,but the hospitalization expenses structure needs to be optimized,and the medical consumables fees has the greatest influence on the hospitalization expenses.It is suggested that the management of medical consumables should be further strengthened,at the same time,it is suggested that the clinical pathway of lung cancer operation should be improved and the proportion of medical service items that embody the technical value of medical staff should be increased.
