摘要
目的探讨甲状腺乳头状癌(PTC)超声造影血流灌注定量参数与微血管密度(MVD)的相关性。方法选取2019年5月至2021年5月我院收治的75例PTC患者为研究对象。所有患者均行超声造影血流灌注,记录血流灌注定量参数[达峰时间(TTP)、平均渡越时间(MTT)、峰值强度(PI)及曲线下面积(AUC)];术后对甲状腺病理组织进行免疫组化染色以获取MVD;分析血流灌注定量参数与MVD的相关性。结果PTC结节组织的PI、AUC及MVD低于正常组织(P<0.05)。不同肿瘤大小患者的PI、AUC及MVD比较,差异具有统计学意义(P<0.05)。淋巴结转移患者的PI、AUC及MVD高于淋巴结未转移患者(P<0.05)。Spearman相关性分析显示,PI、AUC与MVD呈正相关(P<0.05)。结论PTC超声造影血流灌注定量参数与MVD密切相关,不仅可用于术前诊断及评估肿瘤血管生成情况,还能预测肿瘤侵袭及转移情况,具有较高的临床应用价值。
Objective To investigate the correlation between blood perfusion quantitative parameters of contrast-enhanced ultrasound and microvessel density(MVD)in papillary thyroid carcinoma(PTC).Methods A total of 75 PTC patients admitted in our hospital from May 2019 to May 2021 were selected as the study objects.All patients underwent blood perfusion of contrast-enhanced ultrasound,and the blood perfusion quantitative parameters[time to peak(TTP),mean transit time(MTT),peak intensity(PI)and area under the curve(AUC)]were recorded;MVD was obtained by immunohistochemical staining of thyroid pathological tissue after operation;the correlation between blood perfusion quantitative parameters and MVD was analyzed.Results The PI,AUC and MVD in the PTC nodules tissues were lower than those in the normal tissues(P<0.05).There were statistically significant differences in PI,AUC and MVD among the patients with different tumor sizes(P<0.05).PI,AUC and MVD in patients with lymph node metastasis were higher than those in the non-lymph node metastasis(P<0.05).Spearman correlation analysis showed that PI and AUC were positively correlated with MVD(P<0.05).Conclusion The blood perfusion quantitative parameters of PTC contrast-enhanced ultrasound are closely related to MVD,it can not only be used for preoperative diagnosis and evaluation of tumor angiogenesis,but also predict the invasion and metastasis of tumor,which has high clinical application value.
