摘要
目的探讨CT引导下经皮肺穿刺活检术用于尘肺病患者的安全性及相关影响因素。方法选择2020年1月至2022年12月本院收治的80例疑似尘肺病患者作为研究对象。所有患者均行CT引导下经皮肺穿刺活检术,统计其诊断结果及并发症发生情况;分析CT引导下经皮肺穿刺活检术安全性的影响因素。结果CT引导下经皮肺穿刺活检术病理检查对尘肺病的诊断准确率为98.75%;80例行CT引导下经皮肺穿刺活检术患者中,气胸发生率为32.50%,肺出血发生率为18.75%。合并慢性阻塞性肺疾病(COPD)、穿刺针停留时间及病灶取材数量均为CT引导下经皮肺穿刺活检术患者发生气胸的独立危险因素(P<0.05)。病灶直径、穿刺深度、穿刺针停留时间及病灶取材数量为CT引导下经皮肺穿刺活检术患者发生肺出血的独立危险因素(P<0.05)。结论CT引导下经皮肺穿刺活检术对尘肺病患者的诊断准确率较高,但影响诊断安全性的因素复杂,需采取相应措施加以干预,尽可能降低并发症发生率。
Objective To explore the safety and related influencing factors of CT guided percutaneous lung biopsy in patients with pneumoconiosis.Methods A total of 80 patients with suspected pneumoconiosis admitted in our hospital from January 2020 to December 2022 were selected as the research objects.All patients underwent CT guided percutaneous lung biopsy,and the diagnostic results and complications were counted;the influencing factors of the safety of CT guided percutaneous lung biopsy was analyzed.Results The diagnostic accuracy of CT guided percutaneous lung biopsy for pneumoconiosis was 98.75%;among the 80 patients who underwent CT guided percutaneous lung biopsy,the incidence of pneumothorax was 32.50%,and the incidence of pulmonary hemorrhage was 18.75%.The combination of chronic obstructive pulmoriary disease(COPD),puncture needle residence time and the number of lesion samples were independent risk factors for pneumothorax in patients undergoing CT guided percutaneous lung biopsy(P<0.05).The lesion diameter,puncture depth,puncture needle residence time and the number of lesion samples were independent risk factors for pulmonary hemorrhage in patients undergoing CT guided percutaneous lung biopsy(P<0.05).Conclusion CT guided percutaneous lung biopsy has a high diagnostic accuracy for pneumoconiosis patients,but the factors affecting the safety of diagnosis are complex,and corresponding measures should be taken to intervene and minimize the incidence of complications as much as possible.
