摘要
目的 了解儿童照料人对基层儿童保健服务项目的需求情况与影响因素,为提升基层儿童健康管理服务提供参考。方法 2020年11月-2021年5月,采用分层整群抽样的方法抽取贵州省贵阳市主城区40所基层卫生服务机构儿童照料人作为调查对象。通过自制的儿童保健服务需求调查问卷收集调查对象基本情况、对儿童保健服务的需求情况,用二分类logistic回归对基层儿童保健服务项目需求的影响因素进行分析。结果 新生儿期儿童照料人需要的保健服务是健康指导(84.8%)和入户访视(84.0%);满月期需要体格检查(86.5%)和维生素D补充指导(84.5%);婴幼儿期需要体格检查(86.5%)和血红蛋白检查(84.5%);学龄前期需要血红蛋白检查(80.6%)和体格检查(79.2%);学龄期需要注意力测评(58.7%)和口腔卫生保健指导(58.2%);青春期需要超声骨龄检测(40.7%)和青春期儿童情绪指导(38.6%)。logistic多因素回归分析结果显示,居住区域(新生儿期OR=0.38,95%CI:0.27~0.54;满月儿期OR=0.49,95%CI:0.35~0.69;婴幼儿期OR=0.64,95%CI:0.49~0.84;学龄前期体格检查OR=0.52,95%CI:0.37~0.72;学龄前期血红蛋白检查OR=0.57,95%CI:0.42~0.78;学龄期OR=1.98,95%CI:1.59~2.46;青春期OR=2.97,95%CI:2.35~3.76)、家庭平均年收入(新生儿期OR=1.29,95%CI:1.14~1.45;满月儿期OR=1.49,95%CI:1.30~1.70;婴幼儿期OR=1.34,95%CI:1.20~1.49;学龄前期体格检查OR=1.44,95%CI:1.26~1.64;学龄前期血红蛋白检查OR=1.31,95%CI:1.16~1.48;学龄期OR=1.25,95%CI:1.14~1.37;青春期OR=0.70,95%CI:0.63~0.76)、家庭结构(新生儿期OR=2.97,95%CI:2.35~3.76;满月儿期OR=3.80,95%CI:1.60~9.06;学龄前期OR=3.20,95%CI:1.34~7.61)、儿童照料人文化程度(学龄期OR=1.24,95%CI:1.11~1.40)、居住情况(满月儿期OR=2.39,95%CI:1.02~5.27)、儿童照料人年龄(学龄期OR=0.97,95%CI:0.96~0.98)是基层儿童保健服务需求的影响因素(P均<0.01)。结论 学龄期以前,儿童照料人对儿童的生长发育方面相关保健
Objective To understand the demand and influencing factors of child caregivers for primary child healthcare services,so as to provide reference for improving primary child healthcare management services.Methods From November 2020 to May 2021,a stratified cluster sampling method was used to select child caregivers from 40 primary health service institutions in the main urban area of Guiyang city,Guizhou province as survey subjects.A self-made questionnaire on child healthcare service needs was used to collect basic information of the survey subjects and their demand for child healthcare services.Binary logistic regression was applied to analyze the influencing factors of demand for primary child healthcare service items.Results The healthcare services needed by child caregivers during the neonatal period were health guidance(84.8%)and home visits(84.0%);during the one-month period,physical examinations(86.5%)and vitamin D supplementation guidance(84.5%)were required;during the infant period,physical examinations(86.5%)and hemoglobin tests(84.5%)were needed;during the preschool period,hemoglobin tests(80.6%)and physical examinations(79.2%)were necessary;during the school-age period,child caregivers required attention assessments(58.7%)and oral health care guidance(58.2%);during puberty,ultrasound bone age tests(40.7%)and emotional guidance for pubertal children(38.6%)were needed.The results of logistic multifactorial regression analysis showed that the residential area(neonatal period OR=0.38,95%CI:0.27-0.54;one-month period OR=0.49,95%CI:0.35-0.69;infant period OR=0.64,95%CI:0.49-0.84;preschool period OR=0.52,95%CI:0.37-0.72/OR=0.57,95%CI:0.42-0.78;school-age period OR=1.98,95%CI:1.59-2.46;puberty OR=2.97,95%CI:2.35-3.76),average annual family income(neonatal period OR=1.29,95%CI:1.14-1.45;one-month period OR=1.49,95%CI:1.30-1.70;infant period OR=1.34,95%CI:1.20-1.49;preschool period OR=1.44,95%CI:1.26-1.64/OR=1.31,95%CI:1.16-1.48;school-age period OR=1.25,95%CI:1.14-1.37;puberty OR=0.70,95%CI:0.63-0.76),family st
