摘要
目的:观察肾移植术后患者耶氏肺孢子菌肺炎(PJP)的发病情况。方法:选取2021年6月至2024年5月在南华大学附属第二医院进行肾移植术后发生PJP的60例患者作为研究对象。对这些患者临床资料进行回顾性分析。结果:这些患者出院时CD4+辅助/诱导T淋巴细胞、CD3+总淋巴细胞的水平高于入院时(P<0.05),出院时乳酸脱氢酶(LDH)、1-3-β-D-葡聚糖(进行G试验检测)及C反应蛋白(CRP)的水平低于入院时(P<0.05),出院时与入院时白细胞计数(WBC)、降钙素原(PCT)的水平比较,差异无统计学意义(P>0.05)。这些患者病情的治愈率为100.00%,并发症的总发生率为18.33%(11/60)。结论:肺部CT检查联合痰液耶氏肺孢子菌PCR检测可发现PJP患者的早期病变,在合理用药的情况下,可提高其治愈率,但需警惕并发症的发生。
Objective To observe the incidence of Pneumocystis jiroveci pneumonia(PJP)in patients after renal transplantation.Methods:Sixty patients with PJP after renal transplantation in the Second Affiliated Hospital of Nanhua University from June 2021 to May 2024 were selected as the research objects.The clinical data of these patients were retrospectively analyzed.Results The levels of CD4+auxillary/induced T lymphocytes and CD3+total lymphocytes of these patients were higher at discharge than at admission(P<0.05),while the levels of lactate dehydrogenase(LDH),1-3-β-D-glucan(G-test)and C-reactive protein(CRP)were lower at discharge than at admission(P<0.05).There was no significant difference in white blood cell(WBC)count and procalcitonin(PCT)level between discharge and admission(P>0.05).The cure rate for these patients was 100%,and the overall incidence of complications was 18.33%(11/60).Conclusion CT examination of lung combined with PCR detection of Pneumocystis jiroveci in sputum can detect the early lesions of PJP patients,and the cure rate can be improved under the condition of rational drug use,but the occurrence of complications should be vigilant.
