机器人辅助步态训练联合常规训练对严重烧伤后下肢运动及平衡功能障碍患者影响的前瞻性随机对照研究

Effects of robot-assisted gait training combined with conventional training on patients with lower limb motor and balance dysfunction after severe burns:a prospective randomized controlled study

摘要 目的:探讨机器人辅助步态训练联合常规训练对严重烧伤后下肢运动及平衡功能障碍患者的影响。方法:前瞻性选取2021年1月—2023年6月于空军军医大学第一附属医院烧伤与皮肤外科的严重烧伤后下肢运动及平衡功能障碍患者98例,按照随机数表法分为对照组(49例,常规训练干预)和研究组(49例,机器人辅助步态训练联合常规训练干预),比较两组下肢运动能力、平衡功能和步行能力。结果:与干预前比较,两组患者干预4个月左右步长差降低,10 m步行测试、起立行走测试用时缩短,步频、Fugl-Meyer下肢运动功能(FMA-LE)评分升高,稳定时间(TS)、多姿势下稳定测试(PS)降低,稳定极限(LoS)、Berg平衡量表(BBS)评分升高,Holden功能性步行分级(FAC)分级更优(P<0.05)。与对照组比较,研究组干预后左右步长差更低,10 m步行测试、起立行走测试用时更短,步频、FMA-LE评分更高,TS、PS更低,LoS、BBS评分更高,FAC分级更优(P<0.05)。结论:机器人辅助步态训练联合常规训练应用于严重烧伤后下肢运动及平衡功能障碍患者,可提高下肢运动能力、平衡能力和步行能力。 Objective:To investigate the effects of robot-assisted gait training combined with conventional training on patients with lower limb motor and balance dysfunction after severe burns.Methods:98 patients with lower limb motor and balance dysfunction after severe burns who were admitted in the First Affiliated Hospital of the Air Force Medical University from January 2021 to June 2023 were prospectively selected.They were divided into the control group(n=49,conventional training intervention)and the study group(n=49,robot-assisted gait training combined with conventional training intervention)using a random number table.Lower limb motor ability,balance function and walking ability were compared between the two groups.Results:After 4 months of intervention,patients in the two groups had lower difference in left-right stride length,shorter time for 10 m walking test and get-up-and-go test,higher step frequency and lower extremity Fugl-Meyer assessment(FMA-LE)scores,lower stabilization time(TS)and postural stability(PS),higher limit of stability(LoS)and Berg balance scale(BBS)scores,and better function ambulation categories(FAC)grading compared with those before intervention(P<0.05).Compared with the control group,the study group had lower left-right stride length difference,shorter time for 10 m walking test and get-up-andgo test,higher step frequency and FMA-LE scores,lower TS and PS,higher LoS and BBS scores,and better FAC grading after intervention(P<0.05).Conclusion:Application of robot-assisted gait training combined with conventional training to patients with lower limb motor and balance dysfunction after severe burns can improve their lower limb motor ability,balance function and walking ability.
