间歇性充气加压联合不同温度热疗对下肢血液循环的影响

The influence of intermittent pneumatic compression combined with different temperatures hyperthermia on the blood circulation of lower limbs

摘要 目的 探讨间歇性充气加压(IPC)联合不同温度热疗对下肢血液循环的影响。方法 对30只成年雄性新西兰兔行左股骨上端髋部模拟手术以建立血液高凝状态模型,根据随机数字表法将模型分为45℃组(IPC联合45℃热疗)、42℃组(IPC联合42℃热疗)和40℃组(IPC联合40℃热疗),每组10只。于治疗前以及治疗后1、5、10 min完成血流动力学参数的检测。结果 (1)血液峰值流速、平均流速以及血流量:3组治疗后均高于治疗前,且随温度降低逐渐降低(P<0.01);治疗后1 min, 45℃组>42℃组>40℃组(P<0.01);治疗后5 min,血液峰值流速45℃组>42℃组>40℃组(P<0.01),平均流速、血流量45℃组与42℃组比较差异无统计学意义(P>0.05),但两组均显著高于40℃组(P<0.01);治疗后10 min, 3组比较差异均无统计学意义(P>0.05)。(2)心率:治疗后1 min, 3组均较治疗前升高(P<0.05),45℃组>42℃组>40℃组(P<0.01);治疗后5 min, 45℃组和42℃组仍高于治疗前(P<0.01),40℃组与治疗前比较差异无统计学意义(P>0.05);治疗后10 min, 3组与治疗前比较差异均无统计学意义(P>0.05)。治疗后5、10 min, 3组比较差异均无统计学意义(P>0.05)。结论 安全范围内的温度越高,IPC联合热疗的作用越强,血管扩张越显著,血液流速越快,下肢血液循环改善越显著。 Objective To explore the effects of intermittent pneumatic compression(IPC)combined with hyperthermia at different temperatures on blood circulation of the lower extremity.Methods Thirty adult male New Zealand rabbits were performed with the hip simulated surgery of left upper femur in order to establish a model of blood hypercoagulable state,and the model was divided into the 45℃group(IPC combined with 45℃thermotherapy),42℃group(IPC combined with 42℃thermotherapy),and 40℃group(IPC combined with 40℃thermotherapy),according to the method of randomized numerical table,there were 10 rabbits in each group.The hemodynamic parameters were completed before treatment,and at 1,5,and 10 min after treatment.Results ①Peak flow rate,average flow rate,and blood flow volume per minute:after treatment,three groups were all higher than before treatment,and were gradually decreased with the decrease of temperature(P<0.01);at 1 min after treatment,45℃group>42℃group>40℃group(P<0.01);5 min after treatment,the peak blood flow rate of 45℃group>42℃group>40℃group(P<0.01),however,for the mean flow rate and blood flow volume per minute,the difference of 45℃group and 42℃group was not significant(P>0.05),but both groups were higher than 40℃group(P<0.01);at 10 min after treatment,the three groups had no differences,compared with the preoperation(P>0.05).②Heart rate:1 min after treatment,three groups were all increased than before treatment(P<0.05),45℃group>42℃group>40℃group(P<0.01);At 5 min after treatment,the 45℃,42℃ groups were still higher than before treatment(P<0.01),and the difference was not significant in 40℃ group,compared with before treatment(P>0.05);At 10 min after treatment,the differences had no significance among the three groups,compared with the preoperation(P>0.05);At 5,10 min after treatment,the differences among three groups were not statistically significant(P>0.05).Conclusions The temperature within the safe range is higher,the effect of IPC combined with hyperthermia is stro
