摘要
目的 对比膝关节单髁置换与全膝关节置换治疗膝关节单间室骨性关节炎疗效。方法 选择2020年3月至2023年3月固原市人民医院骨科收治的120例膝关节单间室骨性关节炎患者为研究对象,根据治疗方案的不同分为两组,其中60例患者接受膝关节单髁置换术治疗(A组),另外60例患者接受全膝关节置换术治疗(B组)。对比两组手术时间、住院时间、血红蛋白(Hb)水平、血细胞比容(Hct)水平、疼痛视觉模拟量表(VAS)评分、膝关节功能评分系统(HSS)评分、Berg平衡量表评分、起立一行走计时测试(TUG)结果、膝关节活动度。结果 A组手术时间及住院时间均短于B组(P均<0.05)。术后1、3 d,A组Hb、Hct水平均高于B组(P均<0.05),且两组术后3 d均低于术后1 d(P均<0.05);术后1、3 d,A组VAS评分低于B组,且两组术后3 d均低于术后1 d(P均<0.05)。术后1、3个月,A组HSS评分、Berg平衡量表评分、膝关节活动度均高于B组(P均<0.05),但术后3个月高于术后1个月(P<0.05);术后1、3个月,A组TUG测试结果均低于B组(P均<0.05),且术后3个月低于术后1个月(P<0.05)。结论 相比于全膝关节置换术,膝关节单髁置换术可缩短患者手术时间和住院时间,更有助于患者膝关节功能的恢复。
Objective To compare the efficacy of unicondylar knee arthroplasty and total knee arthroplasty in the treatment of knee single compartment osteoarthritis.Methods A total of 120 patients with knee single compartment osteoarthritis admitted to Department of Orthopedics of Guyuan People's Hospital from March 2020 to March 2023 were selected as the research subjects.According to different treatment plans,the patients were divided into two groups.Among them,60 patients received knee single condyle replacement surgery as Group A,and the other 60 patients received total knee replacement surgery as Group B.The surgical time,hospital stay,hemoglobin(Hb)levels,hematocrit(Hct)levels,visual analogue scale(VAS)scores,knee joint function scoring system(HSS)scores,Berg balance scale scores,TUG test results,and knee joint range of motion were compared between the two groups.Results The operation time and hospital stay in group A were shorter than those in Group B(P all<0.05).On postoperative day 1 and 3,the Hb and Hct levels in Group A were higher than those in Group B(P all<0.05),and both groups were lower on postoperative day 3 than on postoperative day 1(P all<0.05);On postoperative day 1 and 3,the VAS scores of Group A were lower than that of the Group B,and both groups had lower VAS scores on postoperative day 3 than on postoperative day 1(P all<0.05).At 1 and 3 months after surgery,the HSS score,Berg balance scale score,and knee joint range of motion in Group A were higher than those in Group B(P all<0.05),but at 3 months after surgery,they were higher than at 1 month after surgery(P<0.05);At 1 and 3 months postoperatively,the TUG test results in Group A were lower than those in Group B(P all<0.05),and at 3 months postoperatively was lower than at 1 month postoperatively(P<0.05).Conclusion Compared with total knee arthroplasty,unicondylar knee arthroplasty can shorten the operation time and hospital stay of patients,and is more conducive to the recovery of knee joint function.
