经鼻高流量湿化氧疗联合气管镜吸痰治疗老年吸入性肺炎的临床疗效

Clinical efficacy of high-flow nasal cannula oxygen therapy combined with tracheoscopy aspiration of sputum in elderly aspiration pneumonia

摘要 目的 观察经鼻高流量湿化氧疗联合气管镜吸痰治疗老年吸入性肺炎患者的临床疗效。方法 将38例老年吸入性肺炎患者随机均分为观察组和对照组。对照组患者给予常规治疗后使用气管镜吸痰;观察组在此基础上联合使用经鼻高流量湿化氧疗。比较两组患者临床症状改善情况、治疗总有效率、血气分析指标变化及不良反应发生情况。结果 与对照组相比,观察组发绀消失、气促消失、肺部啰音消失、抗生素使用及住院时间均缩短,治疗总有效率升高,而不良反应发生率降低(P<0.05)。与治疗前相比,两组治疗后PaO_(2)、SaO_(2)及HCO_(3)^(-)均升高,PaCO_(2)降低(P<0.05),其中观察组改善更加明显(P<0.05)。结论 采用经鼻高流量湿化氧疗联合气管镜吸痰治疗老年吸入性肺炎患者的临床疗效确切,可有效改善血气分析指标,缩短临床症状改善时间,并减少不良反应的发生。 Objective To observe the clinical efficacy of high-flow nasal cannula oxygen therapy combined with tracheoscopy aspiration of sputum in elderly patients with aspiration pneumonia.Methods Thirty-eight elderly patients with aspiration pneumonia were equally randomized into two groups of A and C.The patients accepted tracheoscopy aspiration of sputum after conventional therapy in group C,while those additionally received high-flow nasal cannula oxygen therapy in group A.The improvement of clinical symptoms,total effective rate of treatment,indexes of arterial blood gas and adverse responses were compared between the two groups.Results Compared with group C,the disappeared time of cyanosis,breath shortness and pulmonary rales,duration of antibiotic use and hospital stay were shortened,the total effective rate of treatment was increased,and incidence of adverse responses was decreased in group A(P<0.05).Compared with before,the PaO_(2),SaO_(2) and HCO_(3) were increased,and PaCO_(2) was decreased in both groups after treatment(P<0.05),which were significantly improved in group A(P<0.05).Conclusion The high-flow nasal cannula oxygen therapy combined with tracheoscopy aspiration of sputum has an obvious clinical efficacy in the elderly patients with aspiration pneumonia,which can effectively improve the indexes of arterial blood gas,shorten the improvement time of clinical symptoms and reduce the adverse responses.
