摘要
目的 探讨腹腔镜下袖状胃切除术(laparoscopic sleeve gastrectomy, LSG)联合司美格鲁肽对肥胖合并2型糖尿病(type 2 diabetes mellitus, T2DM)患者糖脂代谢及体重的影响。方法 回顾性选取2021年1月—2023年9月上海市嘉定区中医医院收治的80例肥胖合并T2DM患者的临床资料,根据治疗方法不同分为两组,各40例。A组采取LSG联合二甲双胍治疗,B组行LSG联合司美格鲁肽治疗。比较两组患者空腹血糖、餐后2 h血糖、糖化血红蛋白、甘油三酯、总胆固醇、体重指数及并发症发生率。结果 治疗前,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平比较,差异无统计学意义(P均>0.05)。治疗后,两组空腹血糖、餐后2 h血糖、糖化血红蛋白水平均降低,且B组低于A组,差异有统计学意义(P均<0.05)。治疗前,两组甘油三酯、总胆固醇、体重指数比较,差异无统计学意义(P均>0.05)。治疗后,两组甘油三酯、总胆固醇、体重指数均降低,且B组低于A组,差异有统计学意义(P均<0.05)。两组术后并发症发生率比较,差异无统计学意义(P>0.05)。结论LSG联合司美格鲁肽治疗肥胖合并2型糖尿病效果较好,可有效控制糖脂代谢水平,且安全性高。
Objective To investigate the effect of laparoscopic sleeve gastrectomy(LSG)combined with semaglutide on glucose and lipid metabolism and body weight in obese patients with type 2 diabetes mellitus(T2DM).Methods The clincal data of 80 obese patients with T2DM admitted to Shanghai Jiading District Traditional Chinese Medicine Hospital from January 2021 to September 2023 were retrospectively selected and divided into two groups according to different treatment methods,with 40 cases in each group.Group A was treated with LSG combined with metformin,and group B was treated with LSG combined with semaglutide.The levels of fasting plasma glucose,2-hour postpran-dial plasma glucose,glycated hemoglobin A1c,triglyceride,total cholesterol,body mass index and the incidence of complications were compared between the two groups.Results Before treatment,there were no statistically significant differences in fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c levels between the two groups(all P>0.05).After treatment,the levels of fasting plasma glucose,2-hour postprandial plasma glucose,glycated hemoglobin A1c in the two groups decreased,and those in the group B were lower than the group A,and the differences were statistically significant(all P<0.05).Before treatment,there were no statistically statistically signifi-cant differences in triglyceride,total cholesterol and body mass index between the two groups(all P>0.05).After treat-ment,the triglyceride,total cholesterol and body mass index of the two groups were decreased,and those of group B were lower than those of group A,and the differences were statistically significant(all P<0.05).There was no statisti-cally significant difference in the incidence of postoperative complications between the two groups(P>0.05).Conclusion LSG combined with semaglutide is effective in the treatment of obesity with type 2 diabetes mellitus,which can effectively control glucose and lipid metabolism and has high safety.
