摘要
目的:通过对双源双能CT非线性融合参数的优化,寻找腹部血管图像质量最佳时融合中心、融合宽度参数值,以提高图像质量。方法:回顾性分析80例行全腹部双能CT增强检查患者的影像资料。以线性融合(LB)为对照组,非线性融合(NLB)为实验组,将融合宽度ω固定为0 HU,通过调整融合中心λ的值,将NLB分为4组(NLB1:λ=0 HU,NLB2:λ=150 HU,NLB3:λ=250 HU,NLB4:λ=350 HU)。选取腹部非成对动脉(腹腔干、肠系膜上动脉、肠系膜下动脉)作为研究对象,每例患者比较5组图像中3支血管分叉处感兴趣区(ROI)的CT值、信噪比(SNR)、对比噪声比(CNR),并对NLB1~4组中SNR、CNR最高和最低的两组(NLB1、NLB4组)及LB组进行图像质量评分。结果:NLB1~4组的血管CT值、SNR、CNR均高于LB组(P<0.001)。在腹腔干、肠系膜上动脉NLB1~4组中,NLB1~3组的血管CT值、SNR、CNR均高于NLB4组(P<0.001),NLB1>NLB2>NLB3,但NLB1、NLB2、NLB3三组间的血管CT值、SNR、CNR差异无统计学意义(P>0.05)。在肠系膜下动脉中,NLB1和NLB2组的血管CT值、SNR、CNR高于NLB3和NLB4组(P<0.05),NLB1>NLB2,差异无统计学意义(P>0.05);NLB3组的血管CT值高于NLB4组(P<0.05),NLB3组的SNR、CNR高于NLB4组,但差异无统计学意义(P>0.05)。NLB1~4组的图像噪声均低于LB组,差异有统计学意义(P<0.01),NLB1~4组间比较差异无统计学意义(P>0.05)。NLB1、NLB4、LB三组图像质量主观评分优良率均大于80%,NLB1>NLB4>LB,差异有统计学意义(P<0.05),2位医师间主观评分一致性良好(ICC值均>0.75)。结论:双源双能CT非线性融合技术在腹部血管成像方面具有明显优势,当融合中心λ和融合宽度ω均取0 HU时,对于非大体重的患者,可以显著提高腹部血管图像质量。
Objective:To find the optimal blending center and blending width to improve the image quality of the abdominal artery by using a non-linear blending method.Methods:Retrospectively analyze the imaging data of 80 patients who underwent whole-abdomen dual-energy CT enhancement.Use linear blending images as the control group and non-linear blending images as the experimental group.The blending widthωis fixed at 0HU,and by adjusting the value of blending centerλ,NLB is divided into four groups(NLB1:λ=0HU,NLB2:λ=150HU,NLB3:λ=250HU,NLB4:λ=350HU).Unpaired arteries in the abdomen(coeliac trunk,superior mesenteric artery and inferior mesenteric artery)were selected as the observation objects.CT values,signal-to-noise ratio(SNR)and contrast-to-noise ratio(CNR)of the region of interest(ROI)at the bifurcation of the three vessels in 5 groups of images were compared for each patient.The two groups with the highest and lowest SNR and CNR in NLB1~4(NLB1,NLB4)and LB were scored subjectively.Results:The CT values,SNR and CNR of NLB1~4 groups were higher than those in the LB group(P<0.001).In the NLB1~4 group of coeliac trunk and superior mesenteric artery,the CT values,SNR and CNR of NLB1~3 were higher than NLB4(P<0.001),NLB1>NLB2>NLB3,there was no statistical difference in CT value,SNR and CNR among NLB1,NLB2 and NLB3 groups(P>0.05).In the inferior mesenteric artery,the CT values,SNR and CNR of NLB1 and NLB2 were higher than those of NLB3 and NLB4(P<0.05),NLB1>NLB2,with no statistically significant difference(P>0.05);the CT value of NLB3 was higher than NLB4(P<0.05);the SNR and CNR of NLB3 were higher than NLB4,with no statistically significant difference(P>0.05).The noise of NLB1~4 groups was lower than LB,the difference was statistically significant(P<0.01),but there was no statistically significant difference between NLB1~4 groups(P>0.05).The distinguished achiever ratio of subjective scores of NLB1,NLB4 and LB were all greater than 80%,NLB1>NLB4>LB,the difference was statistically significant(P<0.05).The consistency of
