摘要
目的探讨加速康复外科早期进食对口腔颌面全麻术后患者的影响。方法将2021年5月—2022年2月医院收治的180例口腔颌面外科全麻术后患者,按组间基本特征具有可比性的原则分为对照组与观察组,各90例。观察组患者采取清醒后2 h进食水,4 h进流质饮食;对照组患者采取常规清醒后6 h进食水护理。比较两组患者有无误吸以及不同时段恶心呕吐发生率、口渴评分、咽痛评分、体温、心率、血压变化。结果两组患者均未发生误吸;两组患者术后1 h(T1)时的恶心呕吐发生率比较差异无统计学意义(P>0.05),术后3 h(T2)、术后5 h(T3)时观察组患者恶心呕吐发生率低于对照组(P<0.05);观察两组患者T1、T2、T3时的口渴、咽痛评分,结果显示,观察组三个时点各指标均呈逐渐降低的趋势,对照组呈逐渐升高的趋势,时间之间差异有统计学意义(P_(时间)<0.05);T2、T3时各指标均以观察组低于对照组,差异有统计学意义(P_(组间)<0.05),并且随观察时间延长,组间差异逐渐增大,组间随时间变化的差异有统计学意义(P_(交互)<0.05);观察两组患者术前静息状态T0、T1、T2、T3时的体温、心率、血压水平,结果显示,两组患者不同时段心率、体温呈先降低后升高的趋势,时间之间差异有统计学意义(P_(时间)<0.05);观察组患者在T2、T3时心率、体温变化均小于对照组,差异有统计学意义(P_(组间)<0.05),随着观察时间的延长,组间差异逐渐增大,组间随时间变化的差异有统计学意义(P_(交互)<0.05)。观察组收缩压和舒张压呈先降低后升高的趋势,对照组呈逐渐降低趋势,时间之间差异有统计学意义(P_(时间)<0.05);观察组患者在T2、T3时收缩压和舒张压变化均小于对照组,差异有统计学意义(P_(组间)<0.05),随着观察时间的延长,组间差异逐渐增大,组间随时间变化的差异有统计学意义(P_(交互)<0.05)。结论将加速康复外科早期进食应
Objective To explore the effect of early feeding in accelerated rehabilitation surgery on patients after oral and maxillofacial general anesthesia.Methods A total of 180 patients after oral and maxillofacial surgery general anesthesia admitted to the hospital from May 2021 to February 2022 were divided into a control group and an observation group according to the principle of comparability of basic characteristics between groups,with 90 patients in each group.Patients in the observation group were given water for 2 hours after waking and a liquid diet for 4 hours,patients in the control group were given water for 6 hours after waking.The incidence of nausea and vomiting,thirst score,throat pain score,body temperature,heart rate,and blood pressure at different times were compared between the two groups.Results There were no aspiration occurred in the two groups.There was no significant difference in the incidence of nausea and vomiting between the two groups at 1 h(T1)after surgery(P>0.05).The incidence of nausea and vomiting in the observation group was lower than that in the control group at 3 h(T2)and 5 h(T3)after surgery(P<0.05).The thirst and throat pain scores at T1,T2 and T3 were observed in the two groups.The results showed that in the observation group,all indicators showed a gradual decreasing trend,the control group showed a gradual increasing trend.The difference between time was statistically significant(P<0.05).At T2 and T3,all indicators were lower than the control group by observation group.The difference was statistically significant(P<0.05).And with the extension of observation time,the difference between groups gradually increased.The difference between groups with time was statistically significant(P<0.05).Observe the preoperative resting states T0,T1,T2 of the two groups.The body temperature,heart rate,and blood pressure levels at the time of T3 showed a trend of fi rst decreasing and then increasing the heart rate and body temperature at different time periods in the two groups.The differenc
