摘要
目的:探讨定量CT (QCT)检测股骨假体周围骨密度水平对髋关节置换术疗效的评估价值分析。方法:回顾性分析2019年5月—2021年5月石城县人民医院收治的108例行髋关节置换术患者临床资料,按照Harris评分将其分为A组和B组。A组(Harris评分≥80分) 71例,B组(Harris评分<80分) 37例。QCT检测股骨假体周围骨密度(BMD)水平对两组患者术前、术后1个月及1年的股骨假体周围BMD进行检测,计算骨量变化率并分析Harris评分与骨量变化率的相关性。结果:A组患者术后R1较B组低,R7较B组高,差异有统计学意义(t=2.964、5.317,P<0.05)。A组患者骨量升高率R1、R6较B组低;骨量降低率R2、R3、R4、R5、R7较B组低,差异有统计学意义(t=542.209、548.430;t=498.126、502.064、506.196、597.025、860.134,P<0.05);骨量变化率中R1与Harris评分呈负相关,R7与Harris评分呈正相关,差异有统计学意义(r=-0.471、0.483,P<0.05)。结论:患者行髋关节置换术后,股骨假体周围BMD变化程度越低,疗效就越好。监测行髋关节置换术患者的BMD变化程度,有助于对其疗效的评估。
Objective:To evaluate the value of QCT in detecting bone mineral density around the femoral prosthesis in evaluating the outcome of hip replacement.Methods:Retrospective analysis was made on 108 patients who underwent hip replacement in the hospital from May 2019 to May 2021.They were divided into group A and group B according to Harris score.There were 71 patients in group A(Harris score≥80)and 37 patients in group B(Harris score<80).QCT was used to detect the bone mineral density(BMD)around the femoral prosthesis.The BMD around the femoral prosthesis of the two groups of patients before and 1 and 12 months after the operation was detected.The bone mass change rate was calculated and the correlation between Harris score and the bone mass change rate was analyzed.Results:After operation,R1 in group A was lower than group B,and R7 was higher than group B(t=2.964,5.317;P<0.05).The bone mass increase rates R1 and R6 of group A were lower than those of group B,and the bone mass decrease rates R2,R3,R4,R5 and R7 of group A were lower than those of group B,the difference was statistically significant(t=542.209,548.430;t=498.126,502.064,506.196,597.025,860.134;P<0.05).In the change rate of bone mass,R1 was negatively correlated with Harris score,while R7 was positively correlated with Harris score,with statistically significant difference(r=-0.471,0.483;P<0.05).Conclusion:After hip replacement,the lower the change of BMD around the femoral prosthesis is,the better the curative effect is.Monitoring the degree of BMD changes in patients undergoing hip replacement is helpful to evaluate its efficacy.
