摘要
目的基于CT图像测量不同层面腹主动脉与左侧腰大肌距离(distance between the abdominal aorta and the left psoas major,DALP),分析其影响因素。方法回顾116例正常成人CT和临床资料,测量L_(2)-L_(5)椎间盘层面DALP、左侧腰大肌径线、腰大肌横截面积(cross-sectional area,CSA)、主动脉直径,L_(2)椎体横径及面积、L_(2)和脐水平前腹壁脂肪组织厚度、L_(2)和脐水平皮下脂肪组织面积(subcutaneous fat area,SFA)及腹内脂肪组织面积(visceral fat area,VFA),并计算L_(2)和脐水平VFA/SFA;测量82例患者L_(2)~5椎间盘层面左侧腰大肌放射学密度、密度标准差、放射性密度比(radiographic density ratio,RDR)。分析不同椎间盘层面DALP与其他指标的相关性。结果DALP于L_(2)~L_(5)呈递减趋势。年龄与L_(2)~L_(5)椎间盘层面DALP呈正相关(均P<0.01)。L_(2)~L_(3)椎间盘层面DALP与该层面腰大肌CSA呈负相关(P<0.01),与L_(2)水平VFA/SFA呈正相关(P<0.05);L_(3)~L_(4)椎间盘层面DALP与腰大肌CSA呈负相关(P<0.01),与L_(2)水平VFA/SFA呈正相关(P<0.01);L_(4)~L_(5)椎间盘层面DALP与该层面腰大肌CSA呈负相关(P<0.01),与L_(2)水平VFA/SFA呈正相关(P<0.05)。结论CT图像上,腹主动脉与左侧腰大肌距离与年龄、腰大肌CSA和L_(2)水平VFA/SFA有关;术前行CT检查对OLIF手术具有指导意义。
Objective To measure the distance between the abdominal aorta and the left psoas major(DALP)at different levels based on CT images and analyzed its influential factors.Methods The CT and clinical data of 116 normal adults were retrospectively analyzed.The following measurements were taken:DALP,diameter of the left psoas major,cross-sectional area(CSA)of the psoas major,aortic diameter,transverse diameter and area of the L_(2)vertebral body,thickness of anterior abdominal wall fat at L_(2)and navel level,subcutaneous fat area(SFA),visceral fat area(VFA)at L_(2)and navel level.Additionally,VFA/SFA ratios were calculated at both L_(2)and navel levels.The radiographic density,density standard deviation and radiographic density ratio(RDR)of the left psoas major muscle at three intervertebral disc levels of L_(2)-L_(5)were measured in 82 patients.The correlation between the DALP and other data at three intervertebral disc levels of L_(2)-L_(5)was analyzed,and the regression equation was calculated.Results The DALP decreased from level L_(2)toL_(5).Age was positively correlated with DALP at L_(2)-L_(5)disc level(all P<0.01).The DALP at the L_(2)-L_(3)disc level was negatively correlated with the CSA of the psoas major at the L_(2)-L_(3)disc level(P<0.01),and positively correlated with the VFA/SFA at L_(2)level(P<0.05).The DALP was negatively correlated with the CSA of the psoas major at the L_(3)-L_(4)disc level(P<0.01),and positively correlated with the VFA/SFA at L_(2)level(P<0.01).The DALP was negatively correlated with the CSA of the psoas major at L_(4)-L_(5)disc level(P<0.01),and positively correlated with the VFA/SFA at L_(2)level(P<0.05).Conclusions On CT images,the distance between the abdominal aorta and the left psoas major was influenced by age,CSA of the psoas major and the VFA/SFA at L_(2)level.Preoperative CT examination is of great significance in guiding the OLIF operation.
