鲁拉西酮与帕利哌酮对精神分裂症患者认知功能及脂代谢水平的影响

摘要 目的 研究鲁拉西酮与帕利哌酮对精神分裂症患者认知功能及脂代谢水平影响的临床对比。方法 选取2021年1月至2022年12月上饶市第三人民医院收治的精神分裂症患者80例,采用随机数字表法将其分为A组(鲁拉西酮)和B组(帕利哌酮)各40例,比较两组临床疗效,症状严重程度[阳性和阴性症状量表(PANSS)],认知功能[洛文斯顿认知功能评定测验(LOTCA)],脂代谢指标[甘油三酯(TG)、胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)],不良反应发生情况。结果 治疗后,A组疗效为75.00%,低于B组的95.00%(P<0.05);B组PANSS评分低于A组(P<0.05);A组LOTCA评分均高于B组(P<0.05);两组LDL-C差异无统计学意义(P>0.05),B组TG、TC、HDL-C高于A组(P<0.05);两组不良反应差异无统计学意义(P>0.05)。结论 对于精神分裂症患者,使用帕利哌酮疗效更好,症状改善情况更佳,但鲁拉西酮对脂代谢影响更小,认知功能情况更好,二者安全性相当。 Objective To compare the clinical effects of lurasidone and paliperidone on cognitive function and lipid metabolism in patients with schizophrenia.Methods A total of 80 patients with schizophrenia admitted to Shangrao Third People's Hospital were enrolled between January 2021 and December 2022.According to random number table method,they were divided into group A(lurasidone)and group B(paliperidone),40 cases in each group.The clinical curative effect,symptoms severity[positive and negative symptom scale(PANSS)],cognitive function[Loewenstein occupational therapy cognitive assessment(LOTCA)],lipid metabolism indexes[triglyceride(TG),cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)]and occurrence of adverse reactions were compared between the two groups.Results The curative effect in group A was lower than that in group B(75.00%vs 95.00%,P<0.05).After treatment,scores of PANSS and LOTCA in group B were lower than those in group A(P<0.05).After treatment,there was no significant difference in LDL-C between the two groups(P>0.05),but TG,TC and HDL-C in group B were higher than those in group A(P<0.05).There was no significant difference in adverse reactions between the two groups(P>0.05).Conclusion For patients with schizophrenia,paliperidone has better curative effect and symptoms improvement,while lurasidone has fewer effects on lipid metabolism and better cognitive function,safety of the two drugs is comparable.
