基于ASL技术分析1型发作性睡病患者的脑血流灌注情况

摘要 目的通过核磁共振动脉自旋标记(Arterial spin labeling,ASL)技术探究1型发作性睡病(NT1)患者的脑血流灌注的异常,探讨NT1患者的脑血流改变及可能的发病机制。方法此次研究共纳入自2018年6月-2022年6月于天津医科大学总医院就诊的20名NT1患者及招募的20名年龄、受教育年限匹配的正常志愿者,应用3.0T核磁共振仪对所有受试者进行ASL扫描。采用双样本t检验对两组各脑区的CBF值进行统计学分析,评估所有受试者的脑血流变化。结果与正常对照组相比,NT1组在右侧颞上回、右侧岛叶、左侧颞中回、右侧额上回CBF值减低,而在左侧眶额回、左侧额中回CBF值增加(P<0.001)。结论NT1患者的多个脑区存在CBF值异常,这些脑区的灌注异常可能参与NT1患者疾病的发生,为NT1患者提供了新的神经影像学特征。 Objective To investigate the cerebral blood flow abnormalities in patients with type 1 narcolepsy(NT1)using arterial spin labeling(ASL)technique and to further explore the alterations in cerebral perfusion and potential pathophysiological mechanisms in NT1 patients.Methods This study included a total of 20 NT1 patients and 20 age-and education-matched healthy volunteers recruited from June 2018 to June 2022 at Tianjin Medical University General Hospital.All participants underwent ASL scans using a 3.0T MRI scanner.The CBF values of various brain regions in both groups were statistically analyzed using a paired t-test to evaluate the cerebral blood flow changes in all participants.Results Compared to the healthy control group,the NT1 group showed decreased CBF values in the right superior temporal gyrus,right insula,left middle temporal gyrus,and right superior frontal gyrus,while increased CBF values were observed in the left orbital frontal gyrus and left middle frontal gyrus(P<0.001).Conclusion Abnormal CBF values are present in several brain regions in NT1 patients,and perfusion abnormalities in these brain regions may be involved in the development of the disease in NT1 patients,providing new neuroimaging features in NT1 patients.
