摘要
目的探讨改良Cohen术对儿童输尿管膨出的临床疗效。方法选取2019年11月至2022年8月首都儿科研究所附属儿童医院行改良Cohen术患儿,收集其临床资料,比较手术前后患儿肾盂分离前后径(anterior-posterior renal pelvic diameter,APD)和输尿管直径(ureter diameter,UD)。结果纳入患儿12例,其中男7例、女5例;年龄1~11岁;左侧6例、右侧5例、双侧1例;排尿困难7例,间断性腹痛、腹胀3例,反复泌尿系感染2例。12例患儿均手术成功,排尿困难、腹痛、腹胀及泌尿系感染症状均消失。与术前相比,患儿术后APD及UD较小,差异均有统计学意义(P<0.05)。1例患儿术前合并左侧膀胱输尿管反流Ⅳ级,术后降为Ⅱ级。12例患儿均无新发膀胱输尿管反流及泌尿系感染。结论改良Cohen术利用膨出组织建立抗反流机制,手术操作简便、临床疗效显著、术后并发症少,可用于治疗儿童输尿管膨出,是一种安全、可行的手术方法。
Objective To explore the clinical effect of modified Cohen operation on ureterocele in children.Methods Children with modified Cohen operation in Children's Hospital,Capital Institute of Pediatric from November 2019 to August 2022 were selected,and the clinical data were collected.The anterior-posterior renal pelvic diameter(APD)and ureteral diameter(UD)were compared before and after operation.Results A total of 12 children were selected,including seven males and five females,aged from 1 to 11 years old.There were six cases on the left ureter,five on the right ureter and one on both sides.Dysuria occurred in seven cases,intermittent abdominal pain and abdominal distension in three cases,and recurrent urinary tract infection in two cases.All the children were operated successfully,and the symptoms of dysuria,abdominal pain,abdominal distension and urinary tract infection disappeared.The results of APD and UD after operation were lower than those before operation,and the differences were statistically significant(P<0.05).One child complicated with vesicoureteral reflux(gradeⅣ)in left side before operation was reduced to gradeⅡafter operation.There was no new vesicoureteral reflux and urinary tract infection in all the 12 children.Conclusions The modified Cohen operation is a safe and feasible method to treat ureterocele in children with its simple operation,remarkable clinical effect and few postoperative complications.
