摘要
目的分析肿瘤专科医院住院死亡病例的疾病构成、分布规律及变化趋势,为评价诊疗服务效果提供依据。方法收集甘肃省某三甲肿瘤专科医院2017年1月1日-2022年12月31日618例住院死亡病例信息,对病例的一般情况、病死率及死因构成进行回顾性分析,采用卡方检验和CochranArmatige趋势检验进行组间比较和病死率趋势分析。结果某肿瘤专科医院住院患者病死率为0.36%,男性0.61%,女性0.21%,男性高于女性(χ^(2)=179.384,P<0.05)。各年份病死率不同(χ^(2)=170.419,P<0.05),2017年最高为0.86%;2021年最低,为0.20%。6年间病死率呈逐年下降的趋势(Z=-11.087,P<0.05)。不同年龄组病死率不同(χ^(2)=107.912,P<0.05),且随着年龄增加病死率逐渐上升(Z=7.994,P<0.05)。该院恶性肿瘤死亡病例占所有死亡病例的95.31%;死亡前5位的恶性肿瘤依次为肺癌、胃癌、食管癌、结直肠癌和肝癌。结论肺癌和胃癌、食管癌等消化道恶性肿瘤对甘肃省居民健康危害严重,男性老年人群是重点关注对象。
Objectives This study aims to analyze the disease composition,distribution and trends of inpatient deaths in a cancer hospital,so as to provide a basis for evaluating the effect of diagnosis and treatment services.Methods The information on 618 inpatient deaths from January 1,2017 to December 31,2022 in a certain hospital was collected.The general condition,mortality rate and causes of death of the cases were analyzed retrospectively.The chi-square test and Cochran Armatige trend test were used for inter-group comparison and mortality trend analysis.Results The mortality rate of inpatients in a cancer hospital was 0.36%,with 0.61%for males and 0.21%for females,which was higher in males than in females(χ^(2)=179.384,P<0.05).The year case fatality rate was different(χ^(2)=170.419,P<0.05),with the highest rate of 0.86%in 2017 and the lowest of 0.20%in 2021.The mortality rate of inpatients decreased year by year in 6 years(Z=-11.087,P<0.05).The case fatality rate was different in different age groups(χ^(2)=107.912,P<0.05),and it gradually increased with the increase of age(Z=7.994,P<0.05).Deaths due to malignant tumors accounted for 95.31%of all deaths in the hospital.The top five malignant tumors for death were lung cancer,gastric cancer,esophageal cancer,colorectal cancer and liver cancer.Conclusions Malignant tumors of the digestive tract,such as lung cancer,stomach cancer,and esophageal cancer,pose a serious threat to the health of residents in Gansu Province,with the elderly male population being the focus of attention.
