摘要
目的:为了提高临床对上皮性卵巢癌患者的诊断准确率,探讨上皮性卵巢癌(EOC)患者血清中微小RNA-576-3p(miR-576-3p)和微小RNA-766-5p(miR-766-5p)的表达及临床意义。方法:选取2017年8月至2022年8月期间本院收治的103例EOC患者作为研究组,103例卵巢囊肿患者作为良性对照组,另取103例同期在本院体检的健康者作为健康对照组。测定并分析患者血清miR-576-3p和miR-766-5p表达水平及相关性,并探讨与EOC发生有关的影响因素,受试者工作特征曲线(ROC)评估miR-576-3p和miR-766-5p对EOC的诊断价值。结果:三组PLR和NLR值存在显著性差异(χ^(2)=151.671、69.059,P<0.05);研究组血清miR-576-3p表达水平(0.76±0.22)低于健康对照组(1.02±0.17)和良性对照组(0.92±0.12)(F=57.959,P<0.001),而研究组miR-766-5p表达水平(1.24±0.45)显著高于健康对照组(1.01±0.11)和良性对照组(1.11±0.31)(F=13.227,P<0.001),且治疗后血清miR-576-3p表达水平升高(0.89±0.11)(t=5.364,P<0.001),血清miR-766-5p表达水平降低(1.11±0.35)(t=2.314,P<0.05);FIGO分期III+IV期(0.58±0.16)、发生淋巴结转移(0.60±0.18)以及低分化程度(0.63±0.20)的EOC患者血清miR-576-3p表达水平低于FIGO分期I+II(0.87±0.26)、未发生淋巴结转移(0.85±0.35)以及高分化程度(0.82±0.31)的EOC患者(P<0.05),血清miR-766-5p表达水平变化趋势则与之相反;PLR(OR=2.114)、NLR(OR=1.594)和miR-766-5p(OR=1.730)均为发生EOC的危险因素,miR-576-3p(OR=0.627)则为保护因素(P<0.05);血清miR-576-3p、miR-766-5p、PLR、NLR以及联合检测诊断EOC的曲线下面积(AUC)分别为0.801(95%CI=0.739~0.853)、0.741(95%CI=0.676~0.800)、0.906(95%CI=0.857~0.942)、0.776(95%CI=0.713~0.831)、0.970(95%CI=0.937~0.989),联合检测优于各自单独检测(Z联合vs miR-576-3p=6.149、Z联合vs miR-766-5p=6.300、Z联合vs PLR=3.550、Z联合vs NLR=5.592,P均<0.001)。结论:血清miR-576-3p和miR-766-5p表达水平与EOC的发生存在一定的相关性,联合检测诊断EOC的敏感度
Objective:In order to improve the diagnostic accuracy of patients with epithelial ovarian cancer(EOC)in clinical practice,the expression and clinical significance of microRNA-5766-3p(miR-576-3p)and microRNA-766-5p(miR-766-5p)in the serum of EOC patients were investigated.Methods:A total of 103 EOC patients admitted to our hospital from August 2017 to August 2022 were regarded as the study group,103 patients with ovarian cysts were selected as the benign control group,and another 103 healthy individuals who underwent physical examination at our hospital during the same period were selected as the healthy control group.Measure and analyze the expression levels and correlation of serum miR-576-3p and miR-766-5p in patients,and explore the influencing factors related to the occurrence of EOC.Evaluate the diagnostic value of miR-576-3p and miR-766-5p for EOC using receiver operating characteristic curve(ROC).Results:There was a significant difference in PLR and NLR values among the three groups(χ^(2)=151.671,69.059,P<0.05).The serum miR-576-3p expression level in the study group(0.76±0.22)was lower than that in the healthy control group(1.02±0.17)and benign control group(0.92±0.12)(F=57.959,P<0.001),while the miR-766-5p expression level in the study group(1.24±0.45)was significantly higher than that in the healthy control group(1.01±0.11)and benign control group(1.11±0.31)(F=13.227,P<0.001),and the serum miR-576-3p expression level increased(0.89±0.11)(t=5.364,P<0.001)after treatment,and the expression level of serum miR-766-5p decreased(1.11±0.35)(t=2.314,P<0.05).The serum miR-576-3p expression levels in EOC patients with FIGO stage III+IV(0.58±0.16),lymph node metastasis(0.60±0.18),and poor degree of differentiation(0.63±0.20)were lower than those in EOC patients with FIGO stage I+II(0.87±0.26),no lymph node metastasis(0.85±0.35),and well degree of differentiation(0.82±0.31)(P<0.05),while the trend of serum miR-766-5p expression levels was opposite.PLR(OR=2.114),NLR(OR=1.594),and miR-766-5p(OR=1.730)w
