摘要
目的:分析高频彩超对甲状腺良恶性结节的鉴别诊断价值和声像特征。方法:采用回顾分析法,选取2022年1月~2023年12月本院诊治的60例甲状腺良恶性结节患者为对象。所有患者均进行高频彩超检查,观察诊断结果,对比良恶性结节的声像特征、动脉血流参数。结果:①60例患者共计77个结节,病理检出恶性结节21个、良性结节56个;高频彩超检出恶性结节23个、良性结节54个。以病理检查为金标准,高频彩超的灵敏度为95.24%,特异度为94.64%,符合率为94.81%,正确指数为0.90;②恶性结节以边界模糊、形态不规则、纵横比≥1、内部低回声、侵犯包膜、微小钙化、血流2~3级为主,良性结节与之相反,有统计学差异(P<0.05);③恶性结节的Vmax为(33.50±4.33)cm/s,Vmin为(15.34±2.75)cm/s,RI值为(0.73±0.28),高于良性结节的(27.63±3.59)cm/s、(11.02±2.26)cm/s和(0.46±0.19),有统计学差异(P<0.05)。结论:甲状腺良恶性结节的声像特征和动脉血流参数存在较大差异,可通过高频彩超检查进行鉴别诊断,为临床治疗提供数据支持。
Objective:To analyze the diagnostic value and ultrasound features of high-frequency ultrasound in distinguishing benign and malignant thyroid nodules.Methods:A retrospective analysis was used to select 60 patients with benign and malignant thyroid nodules diagnosed and treated in our hospital from January 2022 to December 2023 as the subjects.All patients underwent high-frequency ultrasound examination to observe the diagnostic results and compare the ultrasound features and arterial blood flow parameters of benign and malignant nodules.Results:①A total of 77 nodules were detected in 60 patients,with 21 malignant nodules and 56 benign nodules detected by pathology;High frequency ultrasound detected 23 malignant nodules and 54 benign nodules.Using pathological examination as the gold standard,the sensitivity of high-frequency ultrasound was 95.24%,the specificity was 94.64%,the conformity rate was 94.81%,and the accuracy index was 0.90.②Malignant nodules were mainly characterized by blurred boundaries,irregular morphology,aspect ratio≥1,low internal echogenicity,capsule invasion,microcalcifications,and blood flow of 2-3 levels,while benign nodules were the opposite,with statistical differences(P<0.05).③The Vmax of malignant nodules was(33.50±4.33)cm/s,Vmin was(15.34±2.75)cm/s,and RI value was(0.73±0.28),which was higher than that of benign nodules(27.63±3.59)cm/s,(11.02±2.26)cm/s,and(0.46±0.19),with statistical differences(P<0.05).Conclusion:There are significant differences in the ultrasound characteristics and arterial blood flow parameters between benign and malignant thyroid nodules,which can be differentiated and diagnosed through high-frequency ultrasound examination,providing data support for clinical treatment.
