摘要
偏头痛是临床常见的神经系统功能障碍性疾病,可表现为两侧颞部、前额、后枕及巅顶头痛,其中慢性、难治性巅顶头痛是偏头痛的疑难类型。目前,临床一线西药能较好地缓解难治性巅顶头痛患者的头痛症状,但存在一定不良反应及药物依赖性;中医药治疗难治性巅顶头痛历史悠久,经验丰富,标本兼顾。全国名中医崔应麟教授深耕脑病三十余年,善从厥阴合病论治难治性巅顶头痛,其认为难治性巅顶头痛的关键病机为浊阴闭塞、清阳不升及髓寒脑逆、乙癸失调,该病在六经辨证中属本虚标实的厥阴合病。笔者总结崔应麟教授从厥阴合病分期论治难治性巅顶头痛的学术思想及临证验案,撷英拾萃,以期丰富难治性巅顶头痛的中医理论和临床实践。
Migraine is a common clinical neurologic dysfunction disease,which can be manifested as bilateral temporal,forehead,posterior occipital and parietal headache.Among them,chronic and refractory parietal headache is a difficult type of migraine.At present,the first-line western medications can effectively alleviate the headache symptoms of patients with refractory parietal headache,but they have certain adverse reactions and drug dependence.Traditional Chinese medicine(TCM)has a long history and rich experience in treating refractory parietal headache,with a specimen balance.Professor Cui Yinglin,a famous traditional Chinese medicine professor,has been ploughing encephalopathy for more than 30 years,and has a lot of experience in treating refractory parietal headache from the perspective of Jueyin combined disease,he believes that the key pathogenesis of refractory parietal headache was turbine-Yin occlusion,lucid Yang failing to rise,brain inversion caused by cold marrow,and the author summarizes Cui Yinglin's academic ideas and clinical case on the treatment of refractory parietal headache from the perspective of Jueyin combined disease,extracting the essentials,with the aim of enriching the Chinese medicine theory and clinical practice of refractory parietal headache.
