摘要
目的探讨内镜下食管静脉曲张套扎术治疗胃底孤立性静脉曲张(IGV)的临床疗效。方法选取2020年6月至2021年12月赣州市第五人民医院收治的45例IGV患者作为研究对象,所有患者均行常规检查,明确食管胃底静脉曲张程度,采用内镜下食管静脉曲张套扎术治疗,比较患者手术前后胃底静脉曲张(GV)、门脉高压性胃黏膜病变(PHG)发生情况及术后并发症发生率、临床疗效。结果术后2周、1个月、3个月、6个月,患者GV、PHG发生率均低于术前,且术后1个月低于术后2周、6个月,差异有统计学意义(P<0.05);患者术后其他时间点GV、PHG发生率两两比较差异无统计学意义。术后1、3、6个月,患者并发症发生率均低于术后2周,治疗总有效率均高于术后2周,且术后6个月并发症发生率、治疗总有效率均低于术后1个月,差异有统计学意义(P<0.05),而术后其他时间点并发症发生率、治疗总有效率两两比较差异无统计学意义。结论内镜下食管静脉曲张套扎术治疗胃底孤立性静脉曲张患者,可降低术后并发症发生率,改善预后效果确切,临床应用价值较高,但术后较易发生再出血现象。
Objective To investigate the clinical efficacy of endoscopic esophageal variceal ligation in the treatment of isolated gastric varices.Methods 45 patients with isolated gastric varices admitted to the Fifth People's Hospital of Ganzhou from June 2020 to December 2021 were selected as the study subjects,routine examination was performed in all patients to determine the degree of esophageal varices,and endoscopic esophageal varices ligation was used to treat the patients.The occurrence of gastric varices(GV)and portal hypertensive gastric mucosal lesions(PHG)before and after surgery,the incidence of postoperative complications,and the clinical efficacy were compared of patients.Results At 2 weeks,1 month,3 months and 6 months after surgery,the incidence of GV and PHG were lower than before surgery,and 1 month after surgery was lower than 2 weeks and 6 months after surgery,the differences were statistically significant(P<0.05);there was no significant difference in the incidence of GV and PHG in pairwise comparisons at other time points after surgery.At 1,3,and 6 months after surgery,the incidence of complications was lower than that at 2 weeks after surgery,and the total effective rate of treatment was higher than that at 2 weeks after surgery,and the incidence of complications and the total effective rate of treatment at 6 months after surgery were lower than those at 1 month after surgery,and the differences were statistically significant(P<0.05);there was no statistically significant difference in the incidence of complications and the total effective rate of treatment in pairwise comparisons at other time points after surgery.Conclusion Endoscopic esophageal variceal ligation in the treatment of isolated gastric varices can reduce the incidence of postoperative complications,and improving prognosis for patients,and have high clinical application value,but it is more prone to rebleeding after surgery.
