摘要
目的 探讨宫腔镜下左炔诺孕酮宫内缓释系统(levonorgestrel-releasing intrautrine system,LNG-IUS)固定术用于防止环下移脱落的有效性。方法 选取2022年3月至2022年12月于广东省妇幼保健院进行宫腔镜下LNG-IUS固定术并完成术后随访的共13例患者,并收集相关资料。结果 患者均顺利完成手术,未出现术中及术后并发症,随访期内,1例患者于术后6月出现LNG-IUS脱落,环脱落率7.7%;3例患者于术后5月到12个月行进一步治疗;12例有症状患者,治疗有效率为75%。结论 宫腔镜下LNG-IUS固定术可有效降低环下移及脱落率,充分评估后选择最佳的手术适合人群,可能进一步提高治疗的有效性。
Objective To investigate the effectiveness of hysteroscopic fixation of levonorgestrel⁃releasing intrautrine system(LNG⁃IUS)for preventing malposition and expulsion of LNG⁃IUS.Method A total of 13 patients who underwent hysteroscopic fixation of LNG⁃IUS and completed postoperative follow⁃up during the period from March 2022 to December 2022 in the Guangdong Women and Children Hospital and Health Institute were recruited,and the clinical characteristics and outcomes were all collected.Results All the patients completed the surgery successfully without preoperative and operative complication,during follow⁃up,one patient expelled LNG⁃IUS 6 months after the surgery,the expulsion rate was 7.7%,3 patient underwent further treatments from 5 to 12 months postoperatively,and in 12 symptomatic patients,the treatment effectiveness rate was 75%.Conclusions Hysteroscopic fixation of LNG⁃IUS can effectively reduce the rate of LNG⁃IUS malpositon and expulsion,and the effectiveness of this treatment may be further improved by selecting the optimal surgical population after thorough evaluation.
