摘要
目的探讨尿道前列腺等离子切除术对前列腺增生患者夜尿症状改善及影响患者术后疗效的因素分析。方法选取2016年5月至2018年7月于石家庄市人民医院就诊的前列腺增生患者158例,随机分为2组,对照组(CG)71例经尿道前列腺电切术,观察组(OG)87例接受经尿道前列腺等离子切除术治疗。比较2组患者术前术后PSA、最大尿流率,比较2组术后夜尿及尿频次数,比较2组术前术后IPSS评分,PABSS评分。比较2组术前术后生活质量评分。采用Logistic分析影响患者术后疗效的相关因素。结果2组患者术后PSA、最大尿流率得到改善,术后夜尿及尿频次数显著降低,患者术后IPSS评分、OABSS评分、QOL评分较术前降低(P<0.05)。多因素Logistic回归结果显示,年龄、尿频、夜尿次数、IPSS评分过高、OABSS评分过高是影响患者疗效的影响因素(P<0.05)。结论经尿道前列腺等离子切除术是治疗前列腺增生的有效手段,而年龄、尿频夜尿次数过多、IPSS及OABSS评分过高是术后影响夜尿症状改善的独立影响因素。
Objective To investigate the effect of transurethral plasmakinetic prostatectomy(TUPKP)on nocturia in patients with benign prostatic hyperplasia(BPH)and its influence on postoperative efficacy.Methods A total of 158 patients with benign prostatic hyperplasia in Shijiazhuang People's Hospital from May 2016 to July 2018 were randomly divided into two groups.71 cases in the control group(CG)underwent transurethral resection of the prostate,and 87 cases in the observation group(OG)underwent transurethral plasmakinetic resection of the prostate.The preoperative and postoperative PSA,maximum urinary flow rate,nocturia and frequency of urination,IPSS score and PABSS score were compared between the two groups.The quality of life scores before and after operation were compared between the two groups.Logistic analysis was used to analyze the related factors affecting the postoperative efficacy of patients.Results The PSA and maximum urinary flow rate of the two groups were improved after surgery,and the nocturia and frequency of urination were significantly reduced after surgery.The IPSS score,OABSS score and QOL score of the two groups were lower than those before surgery(P<0.05).Multivariate Logistic regression showed that age,frequency of micturition,nocturia frequency,IPSS score and OABSS score were the influencing factors affecting the efficacy of patients(P<0.05).Conclusion TUPKP is an effective method for the treatment of benign prostatic hyperplasia.Age,frequent nocturia,high IPSS and OABSS scores are independent factors affecting the improvement of nocturia symptoms after operation.
