摘要
目的探讨单孔腹腔镜全子宫切除术在子宫良性疾病中的应用效果。方法将86例子宫良性疾病患者随机分为对照组和观察组,对照组43例使用常规腹腔镜全子宫切除术,观察组43例使用单孔腹腔镜全子宫切除术。比较两组各项手术指标、术后疼痛程度、美观满意度及并发症发生情况。结果观察组手术时间(105.46±21.32)min长于对照组的(83.37±14.26)min,切口长度(2.67±0.41)cm短于对照组的(4.49±0.52)cm(P<0.05);但两组术中出血量、术后血红蛋白(Hb)变化值、腹腔引流量、术后排气时间、住院时间相当(P>0.05)。观察组术后1、3、7d的视觉模拟评分法(VAS)评分(2.45±0.48)、(1.89±0.31)、(1.21±0.35)分低于对照组的(3.54±0.62)、(2.76±0.45)、(2.08±0.47)分,术后1周的体象障碍自评量表(BIS)评分(12.87±1.74)分、切口美观满意度(CS)评分(21.68±2.06)分高于对照组的(10.05±2.11)、(16.90±2.34)分(P<0.05)。观察组并发症发生率4.65%低于对照组的18.60%(P<0.05)。结论单孔腹腔镜全子宫切除术在子宫良性疾病中的应用效果显著,能显著缩小手术创伤,加快术后恢复速度,减轻术后疼痛,降低并发症发生率,提高对腹部美观的满意度。
Objective To explore the application effect of single-port total laparoscopic hysterectomy in benign uterine diseases.Methods A total of 86 patients with benign uterine diseases were randomly divided into a control group(43 cases,conventional total laparoscopic hysterectomy)and an observation group(43 cases,single-port total laparoscopic hysterectomy).The surgical indicators,postoperative pain intensity,aesthetic satisfaction and complications were compared between the two groups.Results The observation group had longer operation time of(105.46±21.32)min than(83.37±14.26)min in the control group,and shorter incision length of(2.67±0.41)cm than(4.49±0.52)cm in the control group(P<0.05).However,the intraoperative blood loss,postoperative hemoglobin(Hb)changes,intraperitoneal drainage,postoperative exhaust time and hospital stay were similar between the two groups(P>0.05).In the observation group,the VAS scores at 1,3 and 7 d after surgery were(2.45±0.48),(1.89±0.31)and(1.21±0.35)points,which were lower than(3.54±0.62),(2.76±0.45)and(2.08±0.47)points in the control group;the scores of body image scale(BIS)and cutting aesthetic satisfaction(CS)were(12.87±1.74)and(21.68±2.06)points,which were higher than(10.05±2.11)and(16.90±2.34)points in the control group(P<0.05).The incidence of complications was 4.65%in the observation group,which was lower than 18.60%in the control group(P<0.05).Conclusion The application of single-port total laparoscopic hysterectomy has remarkable effects in benign uterine diseases,which can significantly reduce the surgical trauma,accelerate the postoperative recovery,reduce postoperative pain,lower the incidence of complications,and improve the satisfaction on abdominal aesthetics.
