摘要
目的:分析腹腔镜下保留卵巢手术治疗卵巢囊肿蒂扭转患者的临床效果。方法:回顾性分析2021年5月—2023年9月泉州医学高等专科学校附属人民医院收治的86例卵巢囊肿蒂扭转患者的临床资料。根据不同手术方式将其分为观察组(45例)和对照组(41例)。对照组实施常规开腹手术治疗,观察组实施腹腔镜下保留卵巢手术。比较两组围手术期指标,卵巢储备功能、性激素及并发症。结果:观察组手术时间、住院时间均短于对照组,术中出血量少于对照组,首次下床活动时间早于对照组,差异有统计学意义(P<0.05)。术后2周,观察组窦状卵泡数(AFC)、收缩期峰值(PSV)、抗米勒氏管激素(AMH)均高于对照组,差异有统计学意义(P<0.05)。术后2周,观察组卵泡刺激素(FSH)低于对照组,促黄体生成素(LH)及雌二醇(E_(2))均高于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:实施腹腔镜下保留卵巢手术治疗卵巢囊肿蒂扭转患者,临床效果明显,能够保护卵巢储备功能,改善激素水平,促进术后恢复。
Objective:To study the clinical effect of laparoscopic ovarian preservation surgery in the treatment of torsion of ovarian cyst pedicle.Method:The clinical data of 86 patients with torsion of ovarian cyst pedicle admitted to the Affiliated People's Hospital of Quanzhou Medical College from May 2021 to September 2023 were retrospectively analyzed.They were divided into observation group(45 cases)and control group(41 cases)according to different operation methods.The control group underwent routine open surgery,and the observation group underwent laparoscopic ovarian preservation surgery.Perioperative indexes,ovarian reserve function,sex hormones and complications were compared between the two groups.Result:The operation time and hospital stay of the observation group were shorter than those of the control group,the amount of blood loss during operation was less than that of the control group,and the first time of getting out of bed was earlier than that of the control group,the differences were statistically significant(P<0.05).After two weeks of operation,antral follicles(AFC),systolic peak(PSV)and anti-Mullerian hormone(AMH)in observation group were higher than those in control group,the differences were statistically significant(P<0.05).After two weeks of operation,the follicle stimulating hormone(FSH)in observation group was lower than that in control group,luteinizing hormone(LH)and estradiol(E_(2))were higher than those in control group,the differences were statistically significant(P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Laparoscopic ovarian preservation surgery for patients with torsion of ovarian cyst pedicle has obvious clinical effect,which can protect the ovarian reserve function,improve hormone levels,and promote postoperative recovery.
