摘要
目的构建结构方程模型,分析各影响因素对脑卒中后偏瘫患者康复训练依从性的影响。方法采用便利抽样法选取2021年1月至2022年7月于萍乡矿业集团有限责任公司总医院收治的154例脑卒中后偏瘫患者作为研究对象,在干预3个月后对其进行问卷调查,包括一般资料调查问卷、康复训练依从性量表(EAQ)、心理弹性量表(CD-RISC)、领悟社会支持量表(PSSS)、焦虑自评量表(SAS)和抑郁自评量表(SDS)。收集并整理问卷后,采用Amos构建结构方程模型,分析各因素与脑卒中后偏瘫患者康复训练依从性之间的关系。结果计算患者依从指数,结果显示有46例患者表现为依从,96例患者表现为基本依从,12例患者表现为不依从。患者康复训练依从性得分为(38.49±3.17)分,脑卒中后偏瘫患者的心理弹性和社会支持均与康复训练依从性呈显著正相关,而焦虑和抑郁均与康复训练依从性呈显著负相关(均P<0.05);心理弹性和社会支持对脑卒中后偏瘫患者康复训练依从性有较强的直接正向效应,其路径系数分别为1.03和0.10(P<0.05),而负性情绪则对脑卒中后偏瘫患者康复训练依从性有较强的直接负向效应,其路径系数为-0.07(P<0.05)。模型的卡方自由度比(CMIN/DF)和误差均方根(RMSEA)分别为1.374和0.049。结论结构方程模型的拟合结果较好,脑卒中后偏瘫患者的康复训练依从性受多种因素影响,心理弹性、负性情绪和社会支持均是患者康复训练依从性的重要影响因素。临床上可充分发挥患者心理弹性和社会支持作用,并调节患者焦虑和抑郁的负性情绪,能够有效提高患者的康复训练依从性,改善患者预后。
Objective To analyze the influence of various influencing factors on rehabilitation exercise adherence of patients with hemiplegia after stroke with structural equation model established.Methods A total of 154 patients with hemiplegia after stroke who were admitted to the General Hospital of Pingxiang Mining Group Co.,Ltd.from January 2021 to July 2022 were selected by convenience sampling method as study objects.A questionnaire survey was conducted after 3 months of intervention,including general information questionnaire,questionnaire of exercise adherence(EAQ),Connor-Davidson resilience scale(CD-RISC),perceived social support scale(PSSS),self-rating anxiety scale(SAS)and self-rating depression scale(SDS).After collecting and sorting out questionnaires,Amos was used to build a structural equation model to analyze the relationship between various factors and rehabilitation exercise adherence of patients with hemiplegia after stroke.Results The patients'adherence indexes were calculated,and the results suggested that 46 patients showed adherence,96 patients showed basic adherence,and 12 patients showed nonadherence.The rehabilitation exercise adherence score of patients was(38.49±3.17)points.The psychological resilience and social support of patients with hemiplegia after stroke were significantly positively correlated with rehabilitation exercise adherence,while anxiety and depression were significantly negatively correlated with rehabilitation exercise adherence(all P<0.05).Psychological resilience and social support had a strong direct positive effect on rehabilitation exercise adherence of patients with hemiplegia after stroke,with path coefficients of 1.03 and 0.10(P<0.05),while negative emotion had a strong direct negative effect on rehabilitation exercise adherence of patients with hemiplegia after stroke,with path coefficient of-0.07(P<0.05).The Chi-square degree of freedom ratio(CMIN/DF)and root mean square error(RMSEA)of the model were 1.374 and 0.049,respectively.Conclusion The fitting result of struc
