不同剂量维生素K1预防儿童维生素K缺乏性出血的临床研究

摘要 目的探讨预防维生素K缺乏性出血中不同剂量维生素K1的应用价值。方法选取2021年5月至2023年8月广州市花都区妇幼保健院收治的164例维生素K缺乏的患儿为研究对象,依照给药剂量的不同分为治疗组1(55例)、治疗组2(55例)和治疗组3(54例)。治疗组1给予维生素K10.3 mg/次、2组给予维生素K10.5 mg/次、3组给予维生素K11 mg/次,对三组患儿的维生素K缺乏性出血发生率、维生素K水平、凝血指标和凝血酶原前体蛋白指标予以对比、分析。结果三组维生素K缺乏性出血发生率比较,差异无统计学意义(P>0.05);治疗后24 h,治疗组2、治疗组3的维生素K水平均较治疗组1高,差异有统计学意义(P<0.05),但治疗组2和治疗组3比较,差异无统计学意义(P>0.05);治疗第1天,治疗组1的凝血酶原时间(PT)、部分活化凝血活酶时间(APTT)均长于治疗组2和治疗组3,且治疗组2高于治疗组3,差异有统计学意义(P<0.05);治疗第5天,治疗组1的PT和APTT均长于治疗组2和组3,差异有统计学意义(P<0.05);治疗后三组的纤维蛋白原比较,差异无统计学意义(P>0.05);治疗后三组的凝血酶原前体蛋白水平均低于治疗前,治疗组1高于治疗组2和治疗组3,且治疗组2高于治疗组3,差异有统计学意义(P<0.05)。结论在维生素K缺乏性出血的预防中,采用维生素K1补充可获得较好的预防效果,但不同剂量维生素K1的治疗效果不同,推荐使用0.5 mg剂量且以静脉注射的方式给药。 Objective To explore the application value of different doses of vitamin K1 in preventing vitamin K deficiency bleeding.Methods A total of 164 children with vitamin K deficiency admitted to the Huadu Women and Children Hospital in Guangzhou from May 2021 to August 2023 were selected as the study subjects.They were divided into treatment group 1(55 cases),treatment group 2(55 cases),and treatment group 3(54 cases)according to the different dosage of medication.Treatment group 1 was given vitamin K10.3 mg/dose,group 2 was given vitamin K10.5 mg/dose,and group 3 was given vitamin K11 mg/dose.The incidence of vitamin K deficiency bleeding,vitamin K levels,coagulation indicators,and prothrombin precursor protein indicators were compared and analyzed among the three groups.Results There was no statistically significant difference in the incidence of vitamin K deficiency bleeding among the three groups(P>0.05).24 hours after treatment,the vitamin K levels of treatment group 2 and treatment group 3 was higher than that of treatment group 1,with statistical significance(P<0.05).However,there was no statisti call significant between treatment group 1 and treatment group 3(P>0.05).On the first day of treatment,the prothrombin time(PT)and partially activated thromboplastin time(APTT)of treatment group 1 were higher than those of treatment group 2 and 3,and the difference was statistically significant(P<0.05).On the 5th day of treatment,the PT and APTT levels in treatment group 1 were higher than those in treatment group 2 and 3,with statistically significant differences(P<0.05).There was no difference in fibrinogen(FIB)among the three groups after treatment,and the difference was statistically significant(P>0.05).The levels of prothrombin precursor protein in all three groups after treatment were lower than before treatment,and the level in group 1 was higher than that in group 2 and group 3,with statistical significance(P<0.05).Conclusion In the prevention of vitamin K deficiency bleeding,vitamin K1 supplementation can achi
