摘要
目的探讨腹腔镜下空回肠侧侧吻合术治疗2型糖尿病(type 2 diabetes mellitus,T2DM)患者糖脂代谢的影响。方法选取2021年8月—2023年11月上海市嘉定区中医医院收治的88例T2DM患者为研究对象,按照不同的治疗方法分为两组,各44例。对照组采取常规降糖药物治疗,观察组在对照组治疗的基础上实施腹腔镜下空回肠侧侧吻合术。比较两组血糖控制达标率、体重控制达标率、糖脂代谢水平[空腹血糖(fast⁃ing plasma glucose,FPG)、餐后2 h血糖(2-hour postprandial plasma glucose,2 hPG)、甘油三酯(triglycerides,TG)、总胆固醇(total cholesterol,TC)]、血清胰高血糖素样肽-1(glucagon-like peptide-1,GLP-1)及空腹胰岛素(fasting insulin,FINS)水平。结果治疗后,观察组血糖控制达标率、体重控制达标率均高于对照组,差异有统计学意义(P均<0.05)。观察组2 hPG、FPG、TG、TC水平均低于对照组,差异有统计学意义(P均<0.05)。治疗后,观察组GLP-1、FINS水平均优于对照组,差异有统计学意义(P均<0.05)。结论腹腔镜下空回肠侧侧吻合术用于T2DM患者中的效果良好,不仅能改善糖脂代谢,还调节了胰岛素分泌。
Objective To investigate the effect of laparoscopic jejunoileal side-to-side anastomosis on glucose and lipid metabolism in patients with type 2 diabetes mellitus(T2DM).Methods A total of 88 patients with T2DM admit-ted to Shanghai Jiading District Traditional Chinese Medicine Hospital from August 2021 to November 2023 were se-lected as the research objects and divided into two groups according to different treatment methods,with 44 cases in each group.The control group was treated with conventional hypoglycemic drugs,and the observation group was treated with laparoscopic jejunoileal side-to-side anastomosis on the basis of the control group.The Control compli-ance rate of the blood glucose and weight,glucose and lipid metabolism levels[fasting plasma glucose(FPG),2-hour postprandial plasma glucose(2 hPG),triglycerides(TG),total cholesterol(TC)],serum glucagon-like peptide-1(GLP-1),fasting insulin(FINS)levels were compared between the two groups.Results After treatment,the control compli-ance rate of the blood glucose and weight of the observation group were higher than those of the control group,and the differences were statistically significant(both P<0.05).The levels of 2 hPG、FPG、TG and TC in the observation group were lower than those in the control group,and the differences were statistically significant(all P<0.05).After treatment,the levels of GLP-1 and FINS in the observation group were better than those in the control group,and the differences were statistically significant(both P<0.05).Conclusion Laparoscopic jejunoileal side-to-side anastomosis is effective in patients with type 2 diabetes.It can not only improve glucose and lipid metabolism,but also regulate in-sulin secretion.
