摘要
目的观察吲哚菁绿与亚甲蓝联合应用于乳腺癌患者前哨淋巴结活检的效果。方法队列研究。抽取2019年1月至2023年12月于常熟市第一人民医院行前哨淋巴结活检的176例乳腺癌患者,按照随机数字表法分为亚甲蓝组和联合组,每组88例。亚甲蓝组单独应用亚甲蓝进行乳腺癌患者前哨淋巴结活检,联合组采用吲哚菁绿与亚甲蓝联合活检,比较两组前哨淋巴结活检结果。结果亚甲蓝组检出77例乳腺癌患者前哨淋巴结,检出率为87.50%(77/88);联合组检出85例乳腺癌患者前哨淋巴结,检出率为96.59%(85/88)。两组检出率比较差异有统计学意义(P<0.05)。亚甲蓝组检出207枚前哨淋巴结,联合组检出365枚前哨淋巴结,联合组每例患者检出前哨淋巴结数高于亚甲蓝组(P<0.05)。两组均未出现注射部位皮下坏死、感染,联合组仅1例患者出现血压降低,但经升压、扩容等治疗后快速得到缓解。结论乳腺癌患者前哨淋巴结活检中联合应用吲哚菁绿与亚甲蓝的效果较好,检出率更高,且平均每例患者检出前哨淋巴结数量也更多,安全性良好,未出现严重并发症。
ObjectiveTo observe the effect of indocyanine green combined with methylene blue in sentinel lymph node biopsy for breast cancer patients.MethodsA total of 176 patients with breast cancer treated in Changshu No.1 People’s Hospital from January 2019 to December 2023 were selected for the cohort study.And they were divided into the methylene blue group and the combined group by random number table method,with 88 cases in each group.The methylene blue group used methylene blue alone for sentinel lymph node biopsy of breast cancer,and the combined group used indocyanine green and methylene blue together.The sentinel lymph node biopsy results of the two groups were compared.ResultsSentinel lymph nodes were detected in 77 patients with breast cancer in the methylene blue group,with a detection rate of 87.50%(77/88).Sentinel lymph nodes were detected in 85 patients with breast cancer in the combined group,with a detection rate of 96.59%(85/88).There were statistically significant differences in detection rate between the two groups(P<0.05).Two hundred and seven sentinel lymph nodes were detected out in the methylene blue group,and 365 sentinel lymph nodes were detected out in the combined group.The number of sentinel lymph nodes detected per person in the combined group was higher than that in the methylene blue group(P<0.05).No subcutaneous necrosis or infection at the injection site was found in either group;while 1 case in the combination group experienced blood pressure reduction,which was quickly relieved after treatment with vasopressor and volume expansion.ConclusionsApplication of indocyanine green combined with methylene blue in sentinel lymph node biopsy for breast cancer patients has a better effect and a higher detection rate,with high safety but no serious complication;and the number of sentinel lymph nodes detected per case is also more.
