摘要
目的探讨lncRNA-XIST、lncRNA-MALAT1与多囊卵巢综合征(PCOS)不孕症患者胰岛素抵抗及促排卵治疗后妊娠结局的关联性。方法招募2020年2月至2022年1月于唐山市妇幼保健院进行促排卵治疗的125例PCOS不孕症患者(PCOS不孕症组),另选取同期体检健康的女性125名作为对照组。比较两组血清lncRNA-XIST、lncRNA-MALAT1水平及稳态模型评估的胰岛素抵抗指数(HOMA-IR)。采用Pearson相关分析探讨血清lncRNA-XIST、lncRNA-MALAT1水平与HOMA-IR的相关性。采用多因素logistic回归分析探讨影响PCOS不孕症患者促排卵治疗后妊娠结局的因素。结果与对照组比较,PCOS不孕症组血清lncRNA-XIST水平及HOMA-IR较高,lncRNA-MALAT1水平较低,差异有统计学意义(P<0.05)。HOMA-IR与血清lncRNA-XIST水平呈正相关(r=0.689,P<0.001),与血清lncRNA-MALAT1水平呈负相关(r=-0.771,P<0.001)。PCOS不孕症患者经促排卵治疗后累积临床妊娠率为43.20%(54/125)。多因素logistic回归分析结果显示,较高的促黄体生成素(LH)/卵泡刺激素(FSH)比值[OR(95%CI)=1.121(1.005~1.278)]和血清lncRNA-XIST水平[OR(95%CI)=1.252(1.014~1.449)]是促进PCOS不孕症患者治疗后未妊娠的独立危险因素(P<0.05),较高的血清抗米勒管激素(AMH)水平[OR(95%CI)=0.518(0.348~0.771)]、lncRNA-MALAT1水平[OR(95%CI)=0.394(0.238~0.652)]是促进PCOS不孕症患者治疗后获得妊娠的保护因素(P<0.05)。结论PCOS不孕症患者血清lncRNA-XIST呈高表达,lncRNA-MALAT1呈低表达,且两个指标水平与胰岛素抵抗、促排卵治疗后妊娠结局密切相关。
Objective To explore the correlation of long noncoding RNA(lncRNA)X-inactive specific tran-script(XIST)(lncRNA-XIST)and lncRNA-MALAT1 with insulin resistance and pregnancy outcome after ovulation induction therapy in infertile patients with polycystic ovary syndrome(PCOS).Methods A total of 125 infertile patients with PCOS who underwent ovulation induction therapy in Tangshan Maternal and Child Health Hospital from February 2020 to January 2022 were recruited as PCOS infertility group,and other 125 healthy women who underwent physical examination during the same period were selected as control group.The serum levels of lncRNA-XIST and lncRNA-MALAT1,and homeostasis model assessment-insulin resistance(HOMA-IR)index were compared between the two groups.Pearson correlation analysis was used to explore the correlation of serum levels of lncRNA-XIST and lncRNA-MALAT1 with HOMA-IR.Multivariate logistic regression analysis was used to explore the factors affecting the pregnancy outcome after ovulation induction therapy in infertile patients with PCOS.Results The serum levels of lncRNA-XIST and HOMA-IR in the PCOS infertility group were higher than those in the the control group,and the lncRNA-MALAT1 level in the PCOS infertility group was lower,and the differences were statistically significant(P<0.05).HOMA-IR was positively correlated with serum lncRNA-XIST level(r=0.689,P<0.001),and negatively correlated with serum lncRNA-MALAT1 level(r=-0.771,P<0.001).The cumulative clinical pregnancy rate of the PCOS infertility patients after ovulation induction therapy was 43.20%(54/125).The results of multivariate logistic regression analysis showed that higher luteinizing hormone(LH)/follicle stimulating hormonec(FSH)ratio[OR(95%CI)=1.121(1.005-1.278)]and serum level of lncRNA-XIST[OR(95%CI)=1.252(1.014-1.449)]were independent risk factors for promoting non-pregnancy in the PCOS infertility patients after treatment(P<0.05),and higher serum levels of anti-Müllerian hormone(AMH)[OR(95%CI)=0.518(0.348-0.771)]and lncRNA-MALAT1[OR(95
